Purchase Request Form
PLEASE READ BEFORE SUBMITTING PURCHASE REQUEST

Employees or volunteers, must complete a Purchase Request form and seek pre-approval from Department Heads listed below, prior to making any purchases on behalf of the school.

Once submitted, this form will be sent to the appropriate Department Head for final approval, followed by the the Main Office for purchasing. You will be notified once your order has been approved and once it has been ordered.

Please include any special information, vendor options, links or documents below. This will help avoid delayes or mistakes in ordering and receiving the appropriate items
