  • I agree that I am at least 18 years of age. I agree to abide by all guidelines, policies and procedures of the Community Service Work Program (“Program”). I agree to be supervised by Fulton County Animal Services (“FCAS”) and/or LifeLine Animal Project (“LifeLine”) staff and will report directly to the Volunteer Coordinator or my respective shift supervisor. If I fail to abide by the terms of this agreement or am otherwise unable to meet the Program requirements, I may be terminated from the Program. I understand that I may at any time with or without cause be removed from my position as a court-ordered community service worker at the sole discretion of FCAS and/or LifeLine.

     

    I understand that I may be exposed to animals while providing my court-ordered community service for FCAS and/or LifeLine and therefore there exists a risk for personal injury. On behalf of myself and my heirs, personal representatives and executor, I release, discharge, indemnify and hold harmless FCAS and LifeLine and their officers, agents, volunteers and employees from any and all claims, damages, liabilities, causes of action or demands of any nature or cause connected with my community service at the Fulton County Animal Shelter or elsewhere. This includes any costs, attorney’s fees and court costs incurred by FCAS and/or LifeLine or their officers, agents, volunteers, and employees connection with my community service based on damages or injuries which I may incur in any way while performing community service at FCAS or at another location. Such damages are not limited to but may include animal bites, accidents, injuries and personal property damage.

     

    I agree, on behalf of myself and my heirs, personal representatives and executor, to release, discharge, indemnify and hold harmless FCAS, LifeLine, and their officers, agents, volunteers, and employees from any and all damage to or loss of my personal property while providing community service to FCAS.

     

    In connection with my activities as a FCAS and LifeLine community service worker, I further agree to release, discharge, hold harmless, and indemnify FCAS, LifeLine, and their officers, volunteers, employees, and agents from any and all claims, damages, liabilities, causes of action or demands of any nature or cause arising from my driving or riding as a passenger while performing community service or while in any motor vehicle owned, operated or leased by FCAS and/or LifeLine, their officers, employees, volunteers and agents.

     

    I understand that public relations are an important aspect of community service work at FCAS and LifeLine. I, therefore, agree on behalf of myself and my heirs, personal representatives, and executors to allow LifeLine and its agents to use any photographs, video or film taken of me for use in any public relations efforts including brochures, newspaper articles, newsletters, Facebook, website, etc.

  • CSW Program Rules

  • CSW REQUIREMENTS:

    • Community service workers (CSWs) must provide proof of charges requiring you to complete community service at orientation.
    • CSWs must be at least 18 years old.
    • CSWs must not be charged with any violent crimes (e.g. animal abuse, assault, battery).
    • CSWs must fill out a CSW Application, Waiver, and Program Rules packet prior to starting community service. We will provide paper copies at orientation if individuals do not have access to a printer. However, bringing your completed packet with you is recommended.
    • CSWs must attend an orientation before signing up for shifts. The orientation is given once a week at each location; please visit our website (www.lifelineanimal.org/communityservice) for orientation days and times. Orientation lasts for approximately 30 minutes. The orientation will always be given on the same day and time for each location, unless otherwise noted on our website. Registration is not required to attend CSW orientation.

     

    SIGNING-UP FOR CSW SHIFTS:

    • A sign-up sheet will be available with shift times and a respective staff member to report to at the beginning of CSW shifts near the CSW time clock. CSWs can only sign up for shifts up to 2 weeks in advance.
    • CSWs must sign-up for a shift in person. Do not call or email to sign-up for a shift.
    • Once a CSW signs up for a shift, they must commit to working it completely. If something comes up to where they cannot make a shift that they signed up for, they must email communityservice@lifelineanimal.org to be removed, or they can remove their self from the sign-up sheet in-person.
    • CSWs MUST cancel a shift AT LEAST 24 hours in advance of the start of the shift, otherwise you may be asked to leave the program.
    • CSW are allowed one no-show OR one late cancel; any more may result in being asked to leave the program.

     

    WORKING SCHEDULED SHIFTS:

    • CSWs must wear a t-shirt, long pants and closed-toed shoes.
    • CSWs must always keep busy, unless taking a break, or risk termination.
    • CSWs must follow instructions from their supervisor, or risk termination.
    • CSWs must not be under the influence of drugs or alcohol during their shifts. If a CSW is suspected of being under the influence of drugs or alcohol they may risk termination.
    • CSWs must clock-in on time for their shift. They will receive a blank timecard at orientation, where they will write their name and hours required at the top. This timecard MUST stay at the designated location at the shelter at all times. The CSW will then report immediately to the shift’s designated employee for duty.
    • Upon clocking in, CSWs must fill out a name tag and wear this during the entire duration of their shift, so that employees and staff can recognize who they are.
    • If a CSW wants to take a break or lunch, they must notify their staff supervisor as well as clock-in and out for whatever break they take. If a CSW is found to be on break but not clocked-out, they will be terminated.
    • Once a CSW has completed their assigned task, it is their responsibility to report to their supervisor to find out what to do next. Failure to do so may result in termination.
    • CSWs may not interact with clients or animals in any way. If CSWs are approached by a client for assistance or questions, you must immediately direct them to a staff member or volunteer who can help.

     

    COMPLETION OF TOTAL NEEDED COMMUNITY SERVICE HOURS

    • Once a CSW has completed their required number of hours, they must fill out a L.O.C. Request Form, and attach their timecard(s).
    • Blank L.O.C. Request Forms are kept near the CSW time clock. If there are any other required forms that need to be filled out (e.g. from a judge, probation officer, etc.) the CSW must attach those along with their L.O.C. Request Form. Once the CSW fills out the L.O.C. Request Form and submits, they must allow up to 72 hours to receive their Letter of Completion.
    • CSWs must not contact staff unless 72 hours after submitting an L.O.C. Request Form.
    • Once the CSW’s request is received, the respective staff member will verify the total hours completed and fill out the CSW’s Letter of Completion.
    • Letters of Completion will be placed in an envelope with the CSW’s name on front, in the designated basket for the CSW to pick up anytime that is convenient.

     

    FCAS and LifeLine staff have permission to report anything regarding their locations CSW to the CSWs respective judge, probation officer, etc. This can include verification of hours, behavior and performance while working, etc.

    Any such requests the CSW receives from their respective judge, probation officer, etc. should be emailed to communityservice@lifelineanimal.org. 

     

    LifeLine employees reserve the right to refuse anyone to participate in the CSW program based on failure to meet any of the program requirements, as well as having a negative attitude or performance.

     

    Please note: If a CSW wants to work at more than one LifeLine location, they must attend an orientation at each location where they plan to work. The CSW will have separate timecards, Letters of Completion, etc. for each location where they complete community service.
