The Ultimate Solo 401K® Plan is "sponsored" (adopted) by your source of self-employment income. This business is called the "Sponsoring Entity" of the solo 401(k).
Below, please indicate the type of business that will be sponsoring the Plan.
Participating Employers are additional business entities from which you and/or your spouse have self-employed income and from which you will be making contributions to the Plan.
Broad Financial will create a trust which will hold the 401(k) Plan. You may choose a name for the trust if you desire. If you do not enter a name, we will name the trust "(Your initials) Investment Trust" (e.g. if your name was John Doe, we would name the Trust "JD Investment Trust").
The solo 401(k) accountholder listed in the beginning of this application will be the primary Trustee. Please enter the Primary Trustee's physical address.
Trustees are not beneficiaries or inheritors. Trustees have full control of all investments.
Note: The IRS does not allow a Roth IRA to be rolled into a 401(k) plan. However, Roth funds from any other Qualified Retirement Plan may be rolled over to a 401(k).
