Please take a moment to read and check to confirm my terms and conditions. If you have any questions, feel free to email or text me.
I understand that this form is for Charla to get a better understanding of the tattoo I am wanting to get. This does not mean I have made an appointment as of yet. Charla will personally contact me via text or email to discuss booking.
I understand that a sketch of my design will be ready upon arrival of my appointment. I may request to see the design the evening before, at no additional cost. Any changes will be done during my studio time.
I understand that a $100 non refundable booking fee will need to be placed before my spot is considered booked. This does go towards the final total. Appointments are given on a first come first served basis with a booking fee. Booking fees are paid through a business invoice or in person.
I understand that my appointment may be rescheduled up to 2 times, if given at least 48 hours notice. Anything above 2 reschedules or under 48 hours notice will forfeit my booking fee, and a new one will need to be paid in order to rebook.
I understand that skin with rash, acne, sunburn, scars or other medical conditions may not be able to be tattooed. Medication that causes blood thinning can also not be tattooed. Please communicate if you are worried this may be the case. If not your booking fee may be forfeited at your appointment if unable to tattoo.
I understand that Charla will be contacting me through text or email. If no response is given, I understand that my desired appointment may be given to another client. I also understand that communicating with anyone outside of this opens up the risk of scammers, and Charla is not liable.
I understand that the tattoo I’d like may not be finished in the studio time I have requested, and may take multiple sessions.
