This form will serve as a quick reference guide while your pup stays with us as well as a guide to help us get to know him quicker. There will probably be many questions that are not applicable to your pups, please skip them and only answer those questions that are applicable to your pups. There are several required questions for each section too. Please be sure to complete those questions prior to submission.
If multiple pups are in the home, please do as follows for the different types of questions below.
- Open text questions - Enter by pup Name (ex: Spot - loves to play with other dogs but has submissive tendencies, Bear - has some separation anxiety issues but is great with other dogs too)
- Multiple Choice questions
- The "How many pups are in the home question?" determines whether the questions for Pup #2 need to be displayed. If 1 is selected, they will not. If 1 is not selected, they will.
- Please answer all first questions for the pup named in the table in the Pup #1 line.
- Please answer all second questions for the pup named in the table in the Pup #2 line.
- Select all choices that are applicable to each pup in each question.
- Add your own answers in the "Other" option, if needed.
Also, when I use the pronoun he or him, I am using it to refer to all dogs whether they are male, female, or multiples.
At the bottom of the form, you have the ability to print the completed form, and if needed, you can also clear the form via the "Clear Form" link.
