Eligibility Questionnaire
  • Oakwood Dog Rescue

Are our dogs suitable for you?
  • Thank you for your interest in adopting a dog from Oakwood Dog Rescue.

This questionnaire is designed to check if the kind of dogs we have at Oakwood Dog Rescue are suitable for you [and your family].

Our dogs are mainly ex-street dogs from Romania and we do not know their history, what breeds are in their mix or how they will settle and take to living in a home in the UK.  They can take more time and patience than UK rescues to decompress and get used to how they should act in the home.

There is a good reason behind all the questions we ask as we need to make sure that you are prepared for the kind of dogs we rehome.

You need to answer all the questions with complete honesty.

You can save your progress at any time by clicking the SAVE button at the bottom of each page and entering your email address so you can be sent a link to return to where you left off.

Please be aware that completing this form may take a while, but PLEASE read each question carefully as it contains important information which you/we need to know.

Depending on your answers there may be extra questions that you will need to answer, these will come up automatically.

At the end of the questionnaire you will submit your form and you will be told that your form has been submitted and instructions on what to do next to progress your application or that some of your answers do not meet our minimum requirements and that we will not be taking your enquiry / application forward.

If you get the latter response, please close your browser as you do not qualify to continue to the Application stage.

  • The kind of dogs we have for rehoming

  • The vast majority of our dogs are ex-street dogs from Romania who have never lived in a home environment before.

As a result, little, if anything, is known about what has happened to them before they arrived with us and what negative experiences they may have had with humans and other animals.

Giving an ex street dog their first home is both incredibly rewarding and can have extra challenges. The dog may:

    • not be able to be touched;
    • be nervous of new objects, sounds and people - much of which we take for granted;
    • try to escape from the home / garden, over or under boundaries and through gaps;
    • not yet have a collar and / or lead trained;
    • require time and patience to get settled into a home; and
    • have not received formal training

Any UK dog that we take as a 'hand-in' are likely to have had issues in the home or a history which is not fully revealed to us, so may have similar issues settling into a new home, so we would also class these dogs as having an unknown history.

  • About 'The Adopter'

  • Unfortunately, we are currently only accpeting applicants over the age of 21. We appreciate your interest and understanding!

  • About you

  • The Adopter will need to sign legal documents to adopt a dog, therefore, The Adopter must have their own personal e-mail address and is not a work email.

All contracts will be sent by email, and must go directly to The Adopter.

There will be no exceptions under any circumstances as these are legally binding documents and agreements.

Contracts cannot be sent to the email address of anyone else, this includes children or partners.

  • If you don't currently have your own personal email address, then you either need to save your responses on this form and go and register for an email address or you need to exit this form as we do require all adopters to have an email address.

Please save your application and return once you have sorted your own personal email address.  Remembering to check and change the email you have entered earlier in the form to ensure that it is your new personal address.

To save - go to the bottom of this page and click 'save'.

  • Your home

  • If you are unable to have dogs in the house, then we are unable to accept an application from you.

If you are unsure, please save the form and check and if you can have pets, amend your answer and continue the form.

If your tenancy agreement states no pets you could always have a chat with them to see if this can be changed.

If you are still unable to have dogs in your home, please close your form as you will not be able to continue your application.

  • Identification

  • PHOTO and ADDRESS identification will need to be provided for all members of the household living at the address who are 16+ and any other adults you need / want to bring with you to a viewing. Scans or high quality photos are required and all text needs to be clearly readable, please do not send in original documents.

This can be one document like a DRIVERS LICENCE which shows both name and current address.  If your drivers licence shows an old address, then you must also send ADDRESS ID as well, to show your current address.

Alterntaively, please use other forms of official photo identification [such as a passport, Disabled Badge or work/student ID card] plus address identification such as a letter from an official source (bank, government office, tenancy agreement etc.) we can accept an image of your address on your internet banking if you don't have any letters / statements.

We only allow those who have provided ID to attend a viewing.

Please DO NOT send these photos now, but you should take the photos and have them ready so they can quickly be sent by email when we request them.

  • Your Family

  • CHILDREN IN THE FAMILY

Due to the kind of dogs we rehome we do not have dogs or puppies that are suitable for homes where there is one or more of the children who are under the age of 10 years old.

This inlcludes members of the family who may only live with your part time or regularly stay over in your home.

It also includes people who are expecting a child under 10 to be living with them in the next year e.g. someone in the home is pregnant / fostering / adopting etc.

If your youngest child is nearing the age of 10, we can accept an application close to their 10th birthday, but we will only finalise an adoption once they reach the minimum age.

We do an assessment of each dog, which is constantly being reviewed, to determine what age children they should be able to cope with in the home.  We state this clearly on their page on our website.  If you have children over 10 years old, you need to check the assessment of the dog you may be interested in and make sure that your child/children are old enough for that dog, otherwise they will not be deemed a match for your family.

  • Dogs suitable for children under 10 years old

If one or more of your children, including those that stay with you part-time, are under the age of 10 years old then the vast majority of our dogs are not suitable for your family.

If we ever get a dog that is suitable for younger children between 5 and 9 years old, we will advertise these on our facebook page and invite only families with younger children to apply.  If you are stil interested in adopting one of our dogs, then we advise you to follow our Facebook page so you can see when [if] we do get any suitable dogs into the rescue.

  • Dogs, Pets & Animals

  • If you are planning to get your dog[s] neutered in the future, we would ask that you wait until they are fully recovered before applying to adopt, so they are in the best position to be able to welcome a new addition to the family.

  • IF YOU ALREADY HAVE DOG[S] IN THE HOME

Normally, we find that adopting dogs into a home with a dog of the opposite sex is the most successful pairing. Dogs of the same sex are more likely to fight and experience problems living together.

If you already have a dog, we will look to match you with a dog of the opposite sex e.g. if you have a male dog already, we will look to match you with a female dog and vice versa.

If you have a male and a female dog already then we will do what we can to ensure a successful adoption, but it may take longer to find a dog that is suitable.

  • Home Security & Escape

  • You will need to send us photos of the entire boundary of your garden and your gate.

We will need photos of the left, right and bottom boundary. These must show the all sections of the walls/fence/hedges etc. including where they attach to your house on both sides. If you have hedges please show photos of any mesh inside the hedges, behind sheds etc. You may need to send us 10+ photos to show all sections of your boundary. If your boundary does not meet the minimum height required for the dog (at least 5ft all the way around), or you have holes in your fences/hedges then your viewing will have to be cancelled. It is important to complete any garden work which is needed before you submit this form to us. We will check your boundary again for you during the video call.

Please DO NOT send these photos now, but you should take the photos and have them ready so they can quickly be sent by email when we request them.

  • Garden Boundary Height

For dogs assessed as a HIGH escape risk, the boundary must be a minimum height of 6ft at the lowest point, including the gate.

For dogs assessed as a LOW/MEDIUM escape risk, the boundary must be a minimum height of 5ft at the lowest point, including the gate.

For dogs assessed as a VERY LOW escape risk, the boundary must be a minimum height of 4ft at the lowest point, including the gate.

For dogs assessed as a NO escape risk, there is no minimum height, but each application will be considered on an individual basis depending on the needs of the dog.

All gates must be padlocked and we will request photos of your garden boundary at a later stage in the application process.

If you do not have secure outside space attached to your home then unfortunately you cannot adopt from us.

  • Any garden boundary which is under the height required for the kind of dog you are looking for, can be heightened with trellis or garden screening but this must be completed before you apply to adopt.

If you need to do work to your garden boundary, please close this form and complete it once your garden work is finished.

  • LONG DISTANCE PROCESS - OVER 30 MILES FROM OAKWOOD - DIRECT ADOPTION

Once we have checked your photo ID's and garden photos we will call you to go over your application form with you in case we have more questions and we will answer any questions you have. We will then arrange a viewing appointment at the centre for you to meet your dog matches.

If we have a dog match at the centre we will book you a virtual homecheck with a virtual viewing appointment at the centre on the following day. There is a £50 (non-refundable and non-transferrable) admin donation payable to book a homecheck with a virtual viewing. This money will form part of your final adoption donation if you choose to adopt a dog. If you decide not to adopt a dog the money will not be refunded to you.

    • When you pay the admin fee we will hold 1 matched dog for you, for 48 hours only. You will need to be prepared to move quickly as during the 48 hours you must have a homecheck and a virtual viewing. If there are multiple dog matches for your family we will only hold 1 match dog for you. However, if other suitable dog matches are at the centre during your viewing appointment you will still be offered the opportunity to meet them and adopt them. If you decide to adopt any dog from us the day of your viewing then the £50 admin donation will form part of your final adoption donation (even if it is not the dog you placed on hold).

  • LOCAL PROCESS - LIVING WITHIN 30 MILES OF OAKWOOD

Once we have checked your photo ID's and garden photos we will call you to go over your application form with you in case we have more questions and we will answer any questions you have. We will then arrange a viewing appointment at the centre for you to meet your dog matches.

All of the adoptive family must attend the VIEWING APPOINTMENT, including any dogs which live with you or visit with your regularly. This will usually take place outside in our field and you should wear appropriate clothing as we cannot move indoors if the weather cold and wet.

You will be required to wear masks at all times and socially distance from our staff members.

You will arrive at the centre and wait outside by your car where you will be greeted by our staff. It is important to follow their instructions at all times regarding moving around the rescue center site. You will bring your own treats with you for the dog (cut up pieces of cheese, chicken and/or hot dogs, the size of your little fingernail or smaller if you can manage it). You will meet the dogs who are a match for your family and discuss their personality and temperament with a member of staff before deciding if you would like to adopt the dog. Please note there are no bathrooms available for use at the centre.

If you decide that you would like to adopt the dog then you will be sent the reservation paperwork by email for you to sign. This must be signed and returned to us by the end of the viewing day along with a bank transfer of £100 reservation donation (non-refundable and non-transferrable) to reserve the dog. The dog will remain at the rescue centre with our staff member while you make preparations for them to live with you permanently. This £100 forms part of the final adoption donation for the dog and stops us from showing the dog to anyone else. *

You will have 48 hours from signing the reservation paperwork to arrange everything you need for the dog such as food, beds, collar, ID tag, insurance etc. We will provide you with a helpful list so you don't forget anything. You understand that if you decide not to adopt the reserved dog at any time all monies already given to Oakwood Dog Rescue are donations and are non-refundable and non-transferrable in any circumstances.

A VIRTUAL HOMECHECK will take place using a video call. This is usually performed using Facebook messenger or WhatsApp and will mean you can show us around your home so we can check the boundary of the garden is secure at the required height for the dog (minimum of 5ft) with a lockable gate. If there are any problems with your garden you will lose your reservation donation and the dog will be offered to someone else on the waiting list. Again, please ensure your garden meets our adoption requirements before you submit an application to adopt. *

The ADOPTION PAPERWORK will be sent to you electronically to sign and you must return the completed adoption paperwork to us along with proof of your pet insurance. You will also transfer any remaining balance of the adoption donation via bank transfer prior before returning to collect the dog from the centre. If you do not complete the adoption within 48hrs of reserving the dog then the dog will no longer be reserved you and you will forfeit your reservation donation and the dog will no longer be reserved to you. *

    COLLECTING YOUR DOG.

We will arrange a further socially distanced meeting in our outdoor space at the centre at an agreed time so you can collect your adoptive dog. You must bring with you the secure crate to transport the dog home, along with an appropriate sized collar and lead, and your dog's new ID tag. We will provide you with your rehoming pack and any other documents belonging to your adopted dog.
