US Squash COVID-19 Pre-Travel Test

COVID-19 Pre-Tournament Proof of Negative Test or Proof of Recovery from Positive Test
Please upload either proof of a negative PCR or Antigen Test taken by the player no more than 2 days before the start of the event. Or, if a player has tested positive for COVID-19 within the 90 days preceding the start of the event upload proof of that test or recovery. Information provided should include:
- Type of test (indicating it is a NAAT or antigen test)
- Entity issuing the result (e.g. laboratory, healthcare entity, or telehealth service)
- Specimen collection date. A negative test result must show the specimen was collected within the 2 days before the first day of the tournament. A positive test result for documentation of recovery from COVID-19 must show the specimen was collected within the 90 days before the first day of the tournament.
- Information that identifies the person (full name plus date of birth)
