Tylar Sunner Digital Consultation Form

This form is the first step to requesting an appointment at Wild Roots Hair Lounge with Tylar Sunner and should take less than five minutes to complete. This digital consultation requires a $50 application fee prior to the response with quote and will be sent in the form of an invoice via email from the booking site VAGARO. This digital consultation form will help give Tylar an idea about your specific hair goals so we can see if you're a good fit for extensions! Please answer COMPLETELY (including all photos) and accurately so we can get your personalized quote and schedule for the appropriate services.  New consultations are reviewed and replied to the upcoming Monday from when they were submitted. Please be aware that it may be 4-6 weeks before a new install can be scheduled should you and Tylar be a good match for services. Filling this out does not by any means secure an appointment. Should you be a good candidate for her chair and move forward with the services,  Tylar will contact you via email (southgasalon@gmail.com) so please check your spam folder. This consultation does not guarantee that you will move forward with any services with Tylar.

    Keeping in mind the overall health and integrity of my client's hair, if Tylar does not feel that your hair is in the proper shape or condition to receive requested services, Tylar has the right to either suggest an alternative service OR refuse to perform service. If at any time during the service, Tylar feels that the integrity of the hair is being compromised, Tylar will stop the service and discuss an alternative service. In addition, if at-home care is not followed resulting in adverse reaction or damage, Tylar reserves the right to refuse reinstall or maintenance. If a client is disrespectful or rude to other guests, the Wild Roots Hair Lounge team, or Tylar, the client may be asked to leave and not return.

    In order to continue providing the best possible scheduling options to my guests, Tylar requires a 48 hours notice should you need to cancel or reschedule your visit. Should a client cancel an appointment with less than 48 hours' notice, a 50% fee of the scheduled service canceled may apply and will be due before a new appointment is scheduled. If client does not show up for a scheduled appointment or calls the day of to cancel, that is considered a NO SHOW. No-shows can be subject to a full-service charge before a new appointment is scheduled. Repeat cancellations or no-shows may not be able to reschedule with any stylist at Wild Roots Hair Lounge. No-shows or last-minute cancellations may result in a loss of deposit.

    The client understands that hair extensions are a luxury service. All quotes are finalized during the consultation and are good for 30 days afterward. A NON-REFUNDABLE 70% deposit is required before any new installs are booked or confirmed. The remaining balance is due by the date of service. Any changes made after the consultation may result in additional costs for installation. All maintenance appointments are an additional, regular cost associated with having extensions. The cost of services may increase over time.

    Tylar strives to offer all guests the highest level of client satisfaction. If you are having challenges with your cut, color, or extension service- let Tylar know within 7 days of your visit and she will discuss how to best correct the issue. Tylar does not offer any refund however if a redo is necessary due to issues on Tylar's part (ie, installation malfunction) correction on that issue at no additional cost within 14 days of service will be suggested. If the issue is due to additional services necessary or more product is needed outside of the scheduled time for the scheduled service is, that may result in extra cost to the client.

    As a professional service provider who values the integrity of your hair, it is highly advised to only use professional hair care products recommended by your stylist for at-home care. Tylar is not liable for any issues with a client's hair as a result from not using recommended care products or using non-professional products (ex: bought online, super market or Sally's).

    Client consents to photos and videos being taken before and after service for documentation and marketing purposes.

    Extensions are attached to your natural hair with either a surgical grade tape adhesive, a copper or aluminum bead, or keratin bond. Any allergies to these materials must be discussed prior to application. Client must acknowledge that hair extensions are very sensitive to heat/color/products and can respond differently than natural hair. While wearing extensions, there is a level of tightness and minimal irritation from added weight tension. While extensions should fit snug, with proper at home care and maintenance they should be free of pain or harm to the natural hair. Removal of extensions is an additional fee.

    Please arrive at ALL appointments with clean, dry hair. Hair must 100% dry prior to service with no conditioner and free of sticky styling products. Do not wash your hair for at least 24 hours after installation. Unless a color service is booked in addition to extension maintenance, a wash and blowout is not included in extension maintenance unless otherwise booked as "Extension Maintenance Blowout".

    Maintenance will be necessary as the natural hair grows, and it is advised for these scheduled appointments to take place every 4-8 weeks for wefts, tapes, or beaded methods and every 10-16 weeks for the bond method (depending on method and natural hair). This timing will be determined during consultation. All appointments should be pre-booked before leaving the salon or booked online on your at www.tylarsunnerhair.com to ensure proper timing. The client understands that it is necessary to keep up with maintenance, as Tylar is not liable for any issues that may occur past the advised timing. If issues occur due to a lack of proper maintenance or at-home care, a charge for removal and a charge for installation may be required even if it occurs within the recommended timing. If matting, slippage or significant detachment issues occur due to improper at-home care and Tylar is not notified prior to the scheduled appointment, Tylar may not have time to perform service and the client will be asked to rebook for a full removal and reinstallation appointment, and will be charged accordingly. A $25 per 1/2 hour detangling fee may apply.

    The client understands that they will be responsible for the proper care of natural hair and extensions to ensure the best possible results. Tylar will provide the client with all at-home care instructions. Tylar will not be held liable for any damage or issues resulting from the lack of proper at-home care by the client. If not cared for properly, matting, slippage and/or detachment may occur. It is necessary to feel your extensions daily to acknowledge if these issues are happening. If at any time you feel that any of these issues are happening please contact Tylar immediately. If any of these issues continue to occur, Tylar may provide you with a recommendation for an alternative method of extensions OR recommend removal to ensure the integrity of your natural hair. Removal or maintenance at home or by anyone other than a certified extension stylist is never recommended and can result in damage to extensions and/or natural hair. If removal is necessary, that appointment cost is not included in the initial installation fee.
