Welcome to the Brittney's Dog House family! Thank you so much for considering Brittney's Dog House LLC to provide daytime exercise, boarding services, companionship, and love for your best friend. Below you will find the client packet which includes a Client Information Form, a Canine Profile Form, Medical Release Form and Dog Care Agreement. The information you will provide on these forms is important and will help us understand the needs of your dog. The forms must be completed in their entirety before your meet & greet. This includes getting your dog's medical information (shot records and vet contact info), Which you may have your vet email to me at: brittneysdoghouse@yahoo.com. Please note that your dog must interact well with other dogs on their first trial daycare day with us in order to stay for the day and/or to return for future visits. This evaluation will be judged at our discretion and is not negotiable. We want your dog to have a positive experience at daycare, and safety is of our utmost concern for all dogs in our care. Boarding and Daycare Requirements

Please Note: I understand that each dog is unique, please don't hesitate to contact me regarding these requirements. I do my best to fit each pups needs while keeping a safe and happy environment for all.

1. Spayed/Neutered by 1 year of age.

It is recommended that all dogs are Spayed/Neutered by 7 months of age. Exceptions can be made for up to 1 year of age or at my discretion. NO Females in Heat.

2. Up To Date on All Vaccinations

Please make sure your dog is fully vaccinated prior to their first visit. The required vaccines are Rabies, Bordetella, & DHLPP. Proof IS Required. It is also recommended that your dog is on Flea Medication to prevent any Flea Issues.

3. Great With Other Dogs

No overly Dominant or Aggressive behavior. Must play well with others. If your pup is more laid back and does not care to partake in play, accommodations can be made so that your pup has a relaxing and calm environment.

4. Potty Trained

Must be trained/ in training to “go” outdoors. Preferably fully potty trained but exceptions can be made for puppies who are still learning.

5. No Major Separation Anxiety

No destructive or disrupted behavior such as: Excessive Chewing, Digging, Clawing, Whining or Barking, or having the habit of trying to escape. No issues with being left alone for short periods of time. If you dog exhibits some of these behaviors an assessment will be made over a period of time to see if the issues can be managed.

6. Preferable Crate Trained (not required for all dogs)

I prefer all dogs to be crate trained although the use of a crate is not always needed. Exceptions can be made for the ones who can be trusted when left unattended (older dogs or more laid back dogs who have no problem relaxing on their own).

MEDICAL RELEASE FORM

This is a required form for all Brittney's Dog House, LLC participants receiving services. First and foremost, the safety and wellbeing of your dog(s) is of the highest importance. Ensuring that your pet remains safe and well cared for is my first responsibility, and as such I take it very seriously. We do our best to have our pet parents screen for pre-existing health conditions, but some factors may be beyond our control. If, in the event, that a medical emergency arises while a dog is in our care, it is imperative that we are immediately able to get them medical treatment at the closest available facility. We will call ahead to the veterinary offices in closest proximity geographically to us to ensure they can handle the emergency present. Your dog will be rushed to the closest available facility for treatment and you will be notified. We notify the owner after we have secured a medical treatment center for the animal to avoid delays that may be caused by emotion on the part of the owner. Our goal is to get your dog medical attention as quickly as humanly possible, and any distractions may interfere with that process. For that reason, it is a requirement to have our pet parents sign this form.

I (the owner) understand that in the event of a medical emergency that Brittney's Dog House, LLC, at its sole discretion, deems to need the immediate attention of a licensed veterinarian, I authorize Brittney's Dog House, LLC to seek medical attention at the closest available veterinary facility. I further agree that I am financially responsible for any medical treatment my pet(s) receives as a result of a medical emergency while attending services provided Brittney's Dog House, LLC.

DOG CARE AGREEMENT

1. I, the dogs owner, further understand that Brittney's Dog House, LLC has relied upon my representation that my dog is in good health and has not injured or shown aggression or threatening behavior to any person or dog in admitting my dog for services at their facility.

2.I, the dogs owner, further understand that their owners, staff, partners and volunteers, will not be liable, financially or otherwise, for injuries to my dog, me or any property of mine while my dog is participating in services provided by Brittney's Dog House, LLC. I hereby release Brittney's Dog House, LLC of any liability of any kind arising from my dogs participation in any and all services provided by Brittney's Dog House, LLC.

3. I, the dogs owner, further understand and agree that any problems with my dog, behavioral, medical or otherwise will be treated as deemed best by staff of Brittney's Dog House, LLC in their sole discretion, and in what they view as the best interest of the animal. I understand that I assume full financial responsibility and all liability for any and all expenses involved in regard to the behavior and health of my dog.

4. I, the dogs owner, further understand that there are risks and benefits associated with group socialization of dogs. I agree that the benefits outweigh the risks and that I accept the risk. I desire a socialized environment for my dog while attending services provided by Brittney's Dog House, LLC and while in their care. I understand that while the socialization and play is closely and carefully monitored by Brittney's Dog House LLC staff to prevent injury, it is still possible that during the course of normal play my dog may receive minor nicks and scratches from roughhousing with other dogs. Any minor injuries to my dog will be pointed out by staff upon pick-up.

5. I, the dogs owner, understand by allowing my dog to participate in services offered Brittney's Dog House, LLC, I hereby agree to allow Brittney's Dog House LLC to take photographs or use images of my pet in print form or otherwise for publication, marketing, and/or promotion.

6. I, the dogs owner, further understand that I am solely responsible, financially or otherwise, for any harm or damage caused by my dog while my dog is attending any services provided by Brittney's Dog House, LLC.

7. I, the dogs owner, understand that if my dog is not picked up on time or by a date specified in a separate agreement I hereby authorize Brittney's Dog House LLC to take whatever action is deemed necessary for the continuing care of my dog. I will pay Brittney's Dog House, LLC the cost of any such continuing care upon demand by Brittney's Dog House LLC. I understand that if I do not pick up my animal, Brittney's Dog House, LLC will proceed according to the guidelines provided by Abandonment Animal Laws. I also acknowledge that I will be fully responsible for all attorneys' fees and associated costs if I abandon my dog.
