Make sure your video is no longer than 10 minutes. Make sure you own the right to use all music & images on the video. Make sure your video does not contain inappropriate language, images, or sounds. 

If you are unable to upload your video please email to info@sierraoralsurgery.com. However, we request that you still complete the submission form to qualify for entry.
