This form is here for you to fill out, anonymously, with whatever you think I may benefit from knowing. It can be comments on what I did great, things that you think I can improve on, or both! It can be related to any area of my tattoo practice, wether it's suggestions for flash, ways booking could've been easier, a recap of how your overall experience with me was, or anything else you like. The form will be anonymous, and you're welcome to leave your contact info if you'd like to do so, but it is by no means necessary. I thank you greatly for your feedback, it is incredibly valuable to me in my growth both as a tattooer, and a human :-)
