    Please use this form to communicate needs for any conference attendee requiring special accommodations, including ADA requirements (i.e., wheelchair), dietary (i.e., dairy allergy), or otherwise (i.e., ASL interpreter, seating near electrical outlet for approved device). Our goal is to provide a positive learning experience for all participants, and will work to ensure each attendee's needs are met. We will contact you directly if we need additional information. Please submit this form at least 2 weeks prior to any event. The information provided here will be shared with necessary individuals (such as a Contest Technical Chair, Cluster Chair, or other Championships support personnel). This form should be completed for any accommodations, even if the accommodation is provided by the school or attendee (i.e., service animal, language interpreter).   • NOTE:

    - Accommodations may not be granted if it would present a competitive advantage for any contestant.

    - Documentation is required for some contests (as outlined in the Technical Standards). DO NOT send copies of an IEP, 504, or similar document to the state office. 

    - Wheelchair ramps are provided for the stage and any elevated spaces. Accessible transportation is available upon request and advance notice.

    - ASL interpreters are provided for the Opening and Closing sessions. Schools must provide any interpreters (including foreign language interpreters) to be used in any contest, and notify the state office. Registration fees are waived for any language interpreters provided by a school.

    - If an attendee needs a personal attendant or medical assistant, they must also register for the conference, but their registration fee will be waived. 

    - Service animals are allowed in any public spaces. Attendees with service animals are responsible for alerting the hotel or any sleeping spaces.
