CANCELLATION REQUEST NOTICE
This form is a cancellation request. A Member Services represenative will review your account to ensure minimum commitments have been met. If the conditions of the membership contract have NOT been fullfilled request will be denied and a new request will need to be submitted upon completion of contract conditions.
If your cancellation request is submitted less than 48 hours before your next scheduled payment date you will be subject to a 50% refund processing fee.
