• Applicant must be present

• Complete application (DS-11) in Black Ink

• Bring Passport Photos (we do not take photos)

• Certified birth certificate or previous passport or proof of citizenship

• Photocopy of above (front and back)

• Picture ID (driver license, passport, military)

• Photocopy of ID (front and back)

• Check or Money Order made payable to: US Department of State

• Payment to City of Fircrest: $35, cash, check, debit, or credit card
