WHAT TO BRING TO YOUR APPOINTMENT

• Applicant must be present

• Complete application (DS-11) in Black Ink

• Bring Passport Photos (we do not take photos)

• Certified birth certificate or previous passport or other proof of citizenship

• Photocopy of above (front and back)

• Picture ID (driver license, passport, military)

• Photocopy of ID (front and back)

• A Check or Money Order. Must be made payable to: US Department of State

• Separate $35 payment for City of Fircrest processing fees
