Parents may voluntarily provide a copy of their child’s COVID immunization card by uploading it using this secure form.

This information will only be accessible by school nurses and school administration solely for the purpose of contact tracing and quarantining.

Providing a copy of your child’s vaccination card ahead of time will allow your child to be removed from quarantine lists (as long as they are asymptomatic).
