• MARCH OF THE LIVING - TEEN APPLICATION

  • GENERAL INFORMATION

    COVID brings with it, its own set of challenges.  Policies and protocols will change many times between now our departure.  We ask your indulgence as we navigate this ever changing landscape.  We will be transparent in everything we do.  There are many things we do not yet know, which we hope will become concrete sooner than later, but be assured you will always be kept informed.

    All participants will need to be vaccinated and up to date with a booster, if eligible

    The application will take about 20 - 30 minutes to complete.  As part of the process you will be required to write a short essay (a couple of paragraphs) on "why you want to participate in the March of the Living" or "what you expect to gain from the experience".  You will also need several items to upload - a copy of both sides of your health insurance card; a copy of your vaccination card and a copy of the inside page of your passport (which must expire no earlier than November 30, 2022). 

    You will receive a link to the medical form which must be completed by a physician NOT RELATED TO YOU.  Please do not delay in giving it to your physician and having them return it to us.

    You will need to mail or bring to our office (March of the Living, 5890 S Pine Island Road, Davie, Fl  33328) two passport photos (can be taken in many drugstores)

    You will be asked for a $180 application fee which is entirely refundable until October 15, or when we receive prices from the International March of the Living and you are informed.

    Do not delay in submitting your application, medical form, pictures and fee.  We anticipate the possibility of being limited in our numbers.  Once we have all the items mentioned, you will be given a priority number and an interview time will be arranged.  Then, once we receive all the infomation from the International March of the Living, we will begin to accept participants based on their priority number.  It is to your advange to get a completed application package in as quickly as possible in order to receive a low priority number.

    Be sure to check the dates of our classes as attendance is mandatory.

    Lastly, need based financial assistance is available.  More information on applying will be sent once your completed application package is received.

  • PASSPORT INFORMATION

    If you do not have a passport or it will expire BEFORE November 30, 2022, you must apply for one NOW.  Currently, passports are taking up to 18 weeks.  If you do not have the inside page of a valid passport to upload now, you will need to upload a copy of your completed passport application page or renewal application page when asked

  • FOR STUDENTS WHO ARE ISRAELI CITIZENS OR WHO HAVE AT LEAST ONE PARENT WHO IS AN ISRAELI CITIZEN:  You will need an exemption ("PATOR") from the Israeli military service to depart Israel.  A copy will need to be in our office prior to January 1st.  Contact the Israeli Consulate as quickly as possible

    It is our intention to rely on the medical information you submit below, along with the physician's form (to be sent separatetly) and any supplemental letters from specialists or mental health professionals, in determining your acceptance and continuation in this program.  Omissions or misstatements are at your risk and that of your physician(s) or therapist(s).

    Should you be found to have any condition, mental or physical, that is not fully disclosed on this form, the physician's form or in an accompanyhing letter from an appropriate, qualified medical or mental health professional, you, may, at the sole and absolute discretion of the program, be returned to the USA at your own/family expense, including the expenses of an accompanying adult, or be treated in the country(ies) you are visiting at your own/family expense and there shall be no refund of monies paid.  The leadership of this program, international and local and its sponsoring organizations are hereby released from all responsibility or liability of any kind whatsoever arising out of any aspect of your medical history and mental or physical condition.

    If any changes take place in your medical condition prior to departure, you must immediately submit an explanatory letter, signed by an appropriate, qualified medical or psychological professional, detailing your diagnosis, prognosis and treatment.  Failure to submit such a report may result in your expulsion from the program with no refund of fees.

    In consultation with my parent(s) we certify that I am able to participate on the March of the Living, knowing I will undergo different sleep patterns, frequent and legthy bus travel, late curfews, early wake ups and numberous hours of walking and being on my feet.  We acknowledge the need for the physician's form to be completed and returned to the Broward County March of the Living office within 14 days of this application.

  • PAYMENT AND CANCELLATION POLICIES

    When these become available from the International March of the Living, we will inform all families of the policies.  We thank you in advance for your patience as we navigate the COVID landscape.

  • In addition to the Medical Assessment agreed to above, the undersigned agree to hold the March of the Living, Inc. ("March"), and the Jewish Federation of Broward County ("Federation"), as well as any other organizations, harmless from any claim, loss, damage, bodily injury, personal injuries, including emotional trauma, known or unknown, death, liability or expense (including attorney's fees) which the undersigned participant might sustain/alleged sustain or incur/alleged incur in connection with, as a result of, or by reason of their participating in the March or any of the activities relating thereto, whether based upon the negligence or breach of contract by any of the above mentioned organizations/agencies or their representatives.

    The organizations sponsoring the March operates the tour offered under this program only as agents of the arline, bus operators, hotel facilities, and others which provide the actual arrangements, and are not liable for any act of omission, delay, injury, loss, damage or nonpeformance occurring in connection with these arrangements.

    It is further noted by the undersigned that while all food on the March of the Living is kosher, we cannot always provide for all special dietary needs.

    By placing our names below, the participant and parent(s) agree to abide by all rules and regulations as set forth by the International March of the Living, March of the Living Broward County and/or the Jewish Federation of Broward County.  These rules and regulations will be offered for signature during the interview process.
