School Volunteer Request for Background Check
Anyone 18 years of age or older and willing to volunteer in the grade school on the CLS campus must undergo a criminal background check. School volunteers must be cleared prior to working or serving with grade school students. The first step is to complete this form. Once we have your initial information, you can expect an email with next steps on using Protect My Ministry as the vendor for this service.
