Attention: To specify different PRINT or SOLID Fabric where size, or item require you to do so, use the Additional Item Information form. You will need to select your item below and indicate quantity.  The form will appear depending on the item that requires it. Just complete the form as indicated. 

To use the form:

Click the SAVE button at the end of each row. You can use this form for multiple items and you will not loose the information that you inputted.

The option field can be used to indicate, for example, add a nosewire for a mask.

This form does not collect money only your order and contact information.  Upon sending this form you are agreeing to pay for the items inwhich you have ordered.  Once your order is processed, you will receive a seperate invoice via email.  The invoice will have payment instructions on how to pay for your order.
