To be considered for membership into the Crabb's Prairie Volunteer Fire Department (CPVFD), an applicant MUST complete this application.

Meetings are held on the 1st and 3rd Tuesday of each month from 7:00PM to 9:00PM located at:

Do you understand that successful applicants are required to maintain facial hair in a manner as to ensure a self-contained breathing apparatus mask will form a positive seal on the face?*

Do you understand that successful applicants are required to complete on-line trainings within one year of membership. (These courses are free and details are available during the meetings)*

Do you understand that successful applicants are required to complete an American Heart Association (AHA) Basic Life Support (BLS) for the Health Care Provider.*

The inquiry includes information as to your character, general reputation, personal characteristics, and mode of living.
