  • On behalf of Peerstar, LLC, I am pleased to offer you the position of Certified Peer Specialist`. In this capacity, you will be responsible for providing peer support to your peers. You will report directly to

  • If you answer yes, then on an attached separate sheet of paper, please provide the details of your exclusion (date of exclusion, reason, duration, whether the exclusion has been lifted, date lifted, and copy of documentation proving that you are no longer excluded). Warning: You cannot be employed by Peerstar LLC if you are excluded from participation in these programs. Your continued non-exclusion from these programs is an ongoing condition of employment with Peerstar. If you are not presently excluded from these programs and you later receive notice or have reason to believe that you are or have been excluded, you agree as a condition of employment to notify Peerstar LLC of this fact immediately.

     

    The following outlines your terms of employment:

     

    Job Duties: You will be responsible for performing the duties described in the CPS Job Description, as well as any additional duties assigned by your supervisor. You agree that you will use your best efforts in performing your job and that you will not commit waste, fraud or abuse.

  • Hourly Wage: Payable in accordance with Peerstar’s normal
    payroll practices. Travel time is paid at the minimum wage in effect at the time.

     

    Hours & Location: Part-time (with the potential to go full-time). Hours vary.

     

    Training: If you are not CPS certified, you must attend a two week CPS certification training session (the Pennsylvania approved 75-hour course required to be employed as a CPS) before beginning any work. If you do not successfully complete the training, you will not be eligible for employment. During training, Peerstar will pay you $105 per day, less applicable withholding taxes, to cover the time you spend in training and your meals. If you do not attend all required days of the training, you will be paid a pro-rata amount for the days that you attend. In addition, Peerstar will pay your tuition for CPS certification training and, depending on the location of training, may cover the cost of your hotel. If you leave Peerstar within 90 days of providing billable services you will be responsible to repay the full amount of your tuition. Please speak with Human Resources for further details.

     

    PCB Certification: If you are not yet PCB certified, you must obtain your PCB certification. If you are unable to obtain your PCB certification within 6 months you will no longer be eligible for employment. Peerstar will pay the application fee at the current rate. If you leave Peerstar within 90 days of providing billable services, you will be responsible to repay the full amount of your tuition.

     

    Vacation & Sick Days: Paid Vacation and Sick days are available to full-time staff. PTO for part time is available at the rate of ,03 times billable hours for part-time CPS.

     

    Pay Periods: Peerstar pays employees twice a month (1st and 15th).

     

    Healthcare Benefits for Variable Hour Employees: Variable hour employees must work an average of 30 hours per week during a 6 month lookback measurement period to be eligible for Healthcare Benefits under the Affordable Care Act. You will remain eligible for those benefits through the following 6 month measurement period at which time the hours are calculated again using the past 6 months in order to make a determination regarding your health insurance eligibility. Once you are eligible you may enroll the first date of the following month.

     

    Additional Benefits: Cell phone reimbursement is $40 per month paid to you at the first pay period of the month, provided that you are working billable hours. Employees also are entitled to all benefits mandated by federal, state, or local law.

  • Please note that Peerstar reserves the right to modify, change or eliminate these benefit programs in its discretion, subject to statutory requirements. Moreover, your participation in these benefit programs will be governed by the terms of the individual plans.


    You agree that you will adhere to all Peerstar policies, rules, systems and procedures as shall be in force from time to time, including but not limited to those in the Employee Manual, Code of Conduct, and Orientation Manuals. Peerstar reserves the right to change the provisions of any of these policies at anytime. These policies include the requirements to complete all required documentation for billable hours and to attend all continuing education trainings (at the Company’s expense) needed to maintain certification.

However, please note that your employment will be “at-will,” which means that either you or Peerstar may terminate your employment at any time for any reason, with or without cause. Neither this letter, nor any other written or verbal communication, is intended to create a contract, and your employment is not intended to be for any specific duration. Your “at-will” status may be altered only in a separate written document signed by an authorized Peerstar executive.


    This offer shall expire after fourteen (14) days from the date of this letter. Please indicate your acceptance of this offer of employment by signing and returning to me one (1) signed copy of this letter. This offer constitutes Peerstar’s full and complete offer and supersedes any prior or contemporaneous offers, whether oral or written.
    On your first day of work, you will be required to sign a Confidentiality and Non-Compete Agreement as a condition of your employment.

    1.2 Forms of Identification Driver’s license /Social Security Card/Passport/Birth Certificate Official

    2.Education High school diploma/transcripts/transcripts for GED/ College Diploma- Degree (if Applicable)- Reach out to your School Admin office for transcripts

    3.Direct Deposit Voided blank check or if you use a check card- you must bring written account verification from financial institution with account and routing number and name on it on financial institution letterhead, a print copy of the name of the card with the account information provided

    4.2 Letters of Recommendation and explanation of charges if record exists or under review

    5.Original FBI clearance- Mailed to you directly bring original and receipt of completion I need the original front and back you may keep a copy

    6.Original Child Abuse clearance you may keep a copy

    7.Original CPS certificate- given upon graduation give to RD, if Already Certified reach out to training vendor for copy.
