Informed Consent Waiver and Release
Parent(s) and Student(s) acknowledge they have reviewed the Beach Camp website and have appraised themselves as to the risks associated with offered activities and location. In exchange for acceptance of Parent(s) and Student(s) registration and attendance, Parent(s) and Student(s) agree to assume all risks and hazards which may arise, and agree to release from liability and to defend and indemnify against such liability, Beach Camp LLC, Save Our Shorelines, SSB Properties, Inc., the City of Edmonds, the Marine Mammal Academy, owners of all property upon which Beach Camp is accessed or activities occur, and each of the forgoing’s members, officers, owners, agents, employees, volunteers, and board members, from and against any claim or suit arising out of or related in any way to Parent(s) and Student(s) or their family member’s presence or attendance, including any claims related in any way to Covid 19 or other virus or bacterial illness. By clicking "Submit Form" below, Parent(s) and Student(s) hereby acknowledge understanding all of the provisions above, and freely choosing to accept the provisions of the foregoing paragraphs in exchange for Parent(s) and Student(s) and/or other family members being allowed to access the premises and/or attend or participate in the contemplated event or activities. Parent(s) and Student(s) further agree and attest that this acknowledgement shall have the same force and effect as a written signature.
