  • It is my responsibility to keep my eyes closed, be still, and not use my phone during the entire procedure which may take up to 3 hours.*
  • Every attempt will be made to provide me with the look I desire, but my Lash Technician will place the length & diameter that are safe and won’t be damaging to my natural lashes and my final result may not be what I initially envisioned.*
  • I should not pull off my lash extensions as it may result it permanent damage to my natural lashes and if I need my lash extensions removed I will consult with my Lash Technician for a professional removal.*
  • I will come to my lash appointment makeup free and oil free with no mascara or glue around the eyes. I am required to follow the lash extension aftercare instructions in order to maintain the life of these extensions.*
  • If I have a set done by a previous Lash Technician , I can be refused service unless I am willing to pay a $30 removal fee. I will then be charged for a new set.*
  • I consent to before and after photos for the purpose of documentation, potential marketing and advertising purposes.*
  • Cancelation Policy: After 10 minutes, you'll be charged a $15 fee. After 15 minutes, without communication, your appointment may be canceled. Cancellations must be made at least 24 hours in advance in order to use your deposit towards your rescheduled visit.*
  • COVID Policy: I will NOT come to my appointment if I am sick, have tested positive for COVID, or have been in contact with someone who has tested positive for COVID. I understand that I will be required to leave my appointment if I show up with any symptoms, and will lose my deposit. I will wear a mask for the entire duration of my appointment, with or without vaccination. I will not bring any extra guests, children or pets. FUEGALASH will honor your deposit if you need to reschedule due to illness for 90 days.*
  • ALL DEPOSITS, SERVICES, & PRODUCTS ARE NON REFUNDABLE and NON TRANSFERABLE.*
  • My Lash Technician will take every precaution to minimize or eliminate negative reactions. I agree that by reading, & signing this form, I release Alexa Katz and FUEGALASH from any liabilities, claims or damage of any nature that may result from this treatment & all future appointments or for any conditions that were present, but not disclosed at the time of this procedure that may affect this treatment.*
  • There are some risks of this procedure may be but not limited to eye redness, itching, swelling, stinging, burning, blurry vision, and potential blindness should the adhesive enter the eye or touch the skin, or an allergic reaction can occur.*

  • AFTERCARE

    Avoid any oils, creams or excessive amounts of water for at least 24 hours. Do not sleep on your face for the first 24 hours. Do not use any oil based eye products. Do not use any waterpoof mascara. Use water based makeup/makeup remover. Do not perm, tint, or use an eyelash curler. Brush lashes daily with a clean lash wand given to you at the time of your appointment. Clean your lashes daily for oily skin, or 3-4 days/week for normal skin with an oil-free soap (preferably foam soap). Your Lash Technician can show you how to properly clean your lashes at the time of your appointment. It's normal for you to lose up to about 5 eyelashes a day, due to our natural lash growth cycle. Make sure to follow up for a fill-in 2-3 weeks after a new set. Anything after 3 weeks will require a brand new set.

     

    PRECAUTIONS

    Avoid excessive amounts of high heat, such as blow dryers or smoke near the eyes. Refrain from pulling out lashes, even if they seem to be grown out or coming off. Contact your Lash Technician to professionally remove lashes safely. Failure to do so may result in loss of your natural lashes.

      

    ~A Deposit is required to secure your appointment, it goes towards your total that is due the day of your appointment. REMAINDER BALANCE IS DUE IN CASH ONLY.

    By signing below I fully understand all statements above, I have had sufficient opportunity for discussion to have any questions answered. Please note that I take pride in each set I do! Everything is custom made. I do not rush my services, as this can compromise the integrity of your natural lashes! Each client's foundation will vary. Therefore, I highly suggest you allot enough time and plan your schedule accordingly.

    I accept that it is my obligation to read and comprehend the information contained on this form, as well as to follow the recommendations given to me by my Lash Technician.
