I wish to attend the 9th Annual Addison Mizner Awards sponsored by The Institute of Classical Architecture & Art, FL Chapter (ICAA FL), and I agree to the following:

1.              I am aware and understand that the evening involves the risk of possibly contracting COVID-19. I understand that while the ICAA FL has implemented measures to reduce the risk, the ICAA FL cannot guarantee that I will not become infected due to my participation in the evening.

2.              I hereby expressly waive and release any and all claims, now known or hereafter known, against the ICAA FL, and its officers, directors, and staff, arising out of my participation in the evening. I will not make or bring any such claim against the ICAA FL.

3.              I confirm that I am not experiencing symptoms of COVID-19 (such as cough, shortness of breath, or fever), and do not have a confirmed or suspected case of COVID-19. If I were to experience such symptoms, I will withdraw from the evening. I will also follow all instructions, recommendations, and cautions of the ICAA FL at all times during the evening.

4.                   I confirm that I have received an approved COVID-19 vaccine (double dose of Pfizer or Moderna or single dose of Johnson & Johnson) and I will share proof of vaccination with the ICAA FL.

OR, if not vaccinated,

                     I confirm that I will present proof of a negative Covid-19 test within 48 hours of the event in order to attend the event. I will share proof of this negative test with the ICAA FL chapter via email to director@flclassicist.org ASAP prior to the event.

I will contact the ICAA FL directly with any questions about religious/medical exemptions.

All matters arising out of or relating to this Release shall be governed by and construed in accordance with the laws of the State of Florida.

BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS RELEASE AND THAT I HOLD HARMLESS THE ICAA FL.
