Hurricane Relief Grant Application

Please submit by Friday, November 11.
Through the ASCP Foundation, ASCP provides grants* to pathologists and laboratory professional members and clinical laboratories affected by the crisis following one of the recent hurricanes this season. Grants will be awarded to selected individuals and laboratories residing or located in the affected area. Please submit your application by Friday, November 11th.

To be eligible:
- Applicants must be residents in the area affected by a recent hurricane.
- Applicants must be ASCP members, either pathologists or laboratory professionals.

* Submission of this form does not guarantee that you will be awarded a grant. Grant awards and amounts are within the sole discretion of ASCP, will be based on ASCP member donations to support such grants, and will be limited to no more than $1,000. The ASCP Foundation will review all submissions.
