    Thank you for your interest in booking an appointment with me.

    Please fill out this pre-consultation form to ensure we book the best service to fit your needs and to know you better.

    Once I receive the form, I will reach out by email/text/call within 1-2 days Monday - Saturday.

    Filling out this form with your personal information is for liability

    purposes and to adhere to state board practices.

  • Redo/Refund Policy:

    There are No Refunds on services or products rendered.

    Some transformations require multiple sessions to achieve the desired result, which will be communicated at the time of booking/consultation.

    Please request any adjustments within the first week following the initial service and all efforts will be made to schedule an adjustment appointment as soon as possible. 

    Adjustements will be made on a case by case basis.

  •  Cancellation Policy

    Filling out this form does not guarantee an appointment. 

    Once an appointment is requested a credit card will need to be placed on file to reserve the appointment.

    48 hours notice is required to reschedule or cancel your appointment and avoid cancellation fees.

    There is a $30 cancellation fee incurred for last minute cancellations/reschedule requests with less than 48 hours notice prior to the scheduled appointment time.

    A $50 cancellation fee applies to appointments cancelled/rescheduled within 24 hours of the scheduled appointment.

    Invoices for cancellation fees must be paid in full in order to reserve future appointments.

     

  • General Price Increase Update 

    Thank you for understanding that overhead and supply prices have gone up dramatically over the past two years; since my costs have increased exponentially and/that I need to raise my prices to support the sustainability and fairness of my business.

    If you’d like to move on from our time together, I will miss you and or the opprotunity but, I completely understand, however these prices are firm and non negotiable. 

    My prices have changed slightly to reflect my consistent investment in my own education and industry standard overhead increases.


    I appreciate you, and I will always continue to make those investments to make sure that my skills are at the absolute best they can be, to give you the most amazing results possible.
