By placing my name and/or identification number, I hereby declare myself of being a legal and valid representative of the organisation as stated above, and that all furnished information is true as to my best knowledge. I further hereby declare and covenant with you as follows: a) that the information given above is true and correct, and shall inform Uniqtrans Logistics Sdn Bhd of any changes in writing; b) to settle all out outstanding debts and payables in accordance to agreed payment terms, of which in an event of non-compliance, Uniqtrans Logistics Sdn Bhd reserves the right to refuse delivery or to release further goods at your absolute discretion; c) risk Insurance, including FG, GIT, and Marine Insurance, where applicable, for the cargo during loading, unloading, storage, and transportation, is to be borne by us and that Uniqtrans Logistics Sdn Bhd is not liable to any claims of damages, loss, and/or rejection of cargo which occurs during handling, loading, unloading, storage, and transportation. any insurance polices undertaken by us shall be extended to include Uniqtrans Logistics Sdn Bhd, and its sub-contractors as “Co-Assured” and/or subjected to the “Waiver of Subrogation” clause.
