Opt-in Consent Form

You are required to complete this upon joining the school
We require every student to adhere to the Terms & Conditions of Brighton Ballet School when attending a classes with us. This includes being dressed appropriately for the class that you/your child is attending, arriving on time for class, and conducting yourselves in a manner that is fit for the learning environment at all times. Please refer to our Terms & Conditions, Code of Conduct and Health & Safety Policy for more information. Please read carefully through our requirements and tick each one to confirm you have read and consent to each condition.

I accept that as part of attending a class authorised employees or representatives of Brighton Ballet School may use physical contact as an aid to teaching me/my child and I expressly consent for this to happen. I understand that participation in classes is a physical activity that may incur injury such as falls, sprains, twisted ligaments, and that I accept and assume full responsibility for any loss, damage, injury or adverse consequence suffered as a result of attending a class, excluding loss or damage suffered as the direct consequence of a negligent breach of care of duty by authorised employees and representatives of Brighton Ballet School.*

Child Protection Policy: I understand that if my child is under the age of 14 years old, they will need to be collected from the care of Brighton Ballet School's authorised employees or representatives in person by a parent or guardian (unless written permission is given by a legal parent or guardian prior to their first class)*
