My document's headings (for chapters and other important sections) are in the Heading 1 style.If I have chapter numbers and titles, they're formatted like this: "Chapter 1: Title of Chapter".I used Page Breaks between chapters and other sections of the book.I didn't use manual tabs to indent at the beginning of each paragraph.I didn't press Enter or Return twice between each paragraph.

Your $25 deposit (paid below) covers half the cost of basic book formatting. When Carol Beth Anderson is done with your formatting, she'll provide you with proof files (a print-ready PDF and a generic ePub) to review. Once you approve them, the other half of your payment, plus any add-on costs, will be due. When you've paid in full, she'll also provide any store-specific files you requested (for Amazon/KDP, Apple, Kobo, etc.). By signing below, you agree not to distribute the proof files in any way unless you've paid in full.

If for any reason Carol Beth Anderson can't provide you with proof files by the deadline you've set (which must be at least 7 days after the date you submit this form), or if she refuses you service (which she reserves the right to do for any reason), she will refund your deposit.

Carol Beth Anderson wants to work with you to create a product you'll be happy with. However, by signing below, you acknowledge that customizations and revision rounds are limited (and, when offered, may result in add-on fees) due to the budget price and template-based software Beth uses. She reserves the right to keep your deposit if you cancel after she's already worked on your formatting. If you don't respond to Beth within 14 days of receiving proof files, the agreement will be considered to be canceled by you, and Beth will keep your deposit.
