Your child must be at least 4 years old to join our U4 devision. You will need to provide a birth certificate.

By submitting the form, I, as the parent/guardian of the registrant, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may given under whatever conditions are necessary to preserve the life, limb, or well-being of registrant. By submitting the form, I, the parent/guardian of the registrant, a minor, agree that I and the registrant will abide by the rules of the UFC, its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with soccer and in consideration for the UFC, accepting the registrant for its soccer programs and activities (the "Programs"). I hereby release, discharge and/or otherwise indemnify the UFC, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claimby or on behalf of the registrant as a result of the registrant's participation in the Programs and/or being transported to or from the same, which transportation I hereby authorize.

Cancellation & Refund Policy:

UNIVERSO FC works year-round to provide quality soccer programming for players of all ages. We understand there are times where family or player situations change, like moving out of state, job changing, illness, etc. If that is the case please submit a cancelation request within 30 days in advance to universo.fc@yahoo.com.

If a late cancelation and or refund request is made, Universo FC refund policy is intended to honor requests where appropriate and to protect club membership from incurring expenses on behalf of a player who is no longer willing or able to play.

If the refund request is made the following 15 days, a partial refund will be made, if the refund request is made after 30 days no refund will be made.

Parents are encouraged to advise IMMEDIATELY to UFC if they would like to cancel the registration for they child so UFC could make the necessary arrangements so PayPal do not make any more charges to that account.
