Filling the form does not commit you to anything. It simply starts the conversation, gives you a chance to find out more and an opportunity to be pre-approved, to take home the dog of your dreams.

Disclosure
When applying to foster or adopt (and if I am successful in my application), I agree to the following terms and conditions:
- I agree that the animal is being adopted or fostered for myself and will not be sold, adopted, or given to another party
- I agree that the animal will not be allowed outdoors without supervision
- I agree when taking my dog outdoors, s/he will be on a secure harness & leash. I will not use a retractable or slip leash.
- (If adopting / fostering a cat) I agree that I will not declaw the cat or keep the cat outdoors.
- I agree that this dog is to be a companion animal, not a guard dog. The dog will live inside my home.
- I agree that the animal must be neutered or spayed (if not already done) once they reach an appropriate age
- I agree to care for the animal in a humane manner. This includes supplying adequate food, water, shelter, attention, and medical care.
- I agree that if at any point I cannot keep the animal, I will return him/her to the rescuer, with as much notice as possible
- I understand and agree that the rescuer makes no guarantees about the animal's temperament and is not responsible for future damages or injuries caused by the animal.
- I give the rescuer permission to call or visit my home at a reasonable time, with reasonable notice, to assure that the animal is being properly treated and cared for
- The rescuer will have the right to remove the animal if they feel the environment is considered to be unsafe, unloving or unhealthy for the puppy/dog. The adopter shall be held responsible for the costs incurred due to the necessitated removal of the puppy/dog. This shall include any veterinary costs related to the reason/s for the removal of this puppy/dog.
- If dog is lost or dies, the adopter is obliged to inform the rescuer.
- I agree to keep the rescuer informed of my current home address and phone number
- I agree that all statements I have made on this form are true. If it is found that any statements I have made on this form are not true the adopted animal can be confiscated.
- In case of reasons beyond my control I agree to give a notice of 15 days to return or relocate the dog to the rescuer.
It is acknowledged that the type written signature provided is the same as a hand written signature, thus legal and binding
