    A NOTE ABOUT FORMS:

    In this registration, you will see a number of policies and forms, some requiring a signature, some just an acknowledgement of receipt.

    Q:  Why do we require both AISD/UIL paperwork, and AHSGLAX paperwork?

    A:  AHSGLAX represents Stephen F. Austin High School (SFA) in the Central Texas District of the Texas High School Girls Lacrosse League (THSGLL) and is afforded many of the same privileges as SFA UIL teams, such as equipment use, field access, and use of facilities. This relationship with SFA requires each player and a parent or guardian to read and sign the same AISD and UIL forms, just as if Girls’ Lacrosse were an official AISD and UIL sport.  AHSGLAX has its own paperwork bundle as well, due to its status as a separate legal and operational entity. 

    Inevitably, there will be some overlap between the AISD/UIL paperwork and the AHSGLAX club paperwork.  We have attempted to organize the registration process to minimize duplicative information and effort.  As a result, you'll see the Policies & Forms organized in two parts:  1) AISD/UIL documentation (which is consolidated in a single pdf, and requires just one Parent acknowledgement), and 2) AHSGLAX documentation (which is less voluminous, and requires a signature for each document).

    A FEW NOTES ABOUT DUES/$$$:

    Fall / Spring Program Dues:

    2021-2022 dues for the Season are $795.00.  This payment can be either paid in full or in (3) installments of $265 due at 1) time of registration 2) 10.31.21 and 3) 11.30.21

    Fundraising:

    Because we are self-funded (rather than funded by the AISD / SFA athletics budget), our annual fundraising activities are critical to the program's success. 

    We will communicate with you later in the season regarding our Snapraise campaign (late Fall) and Boys/Girls Lax Fiesta Party (early Spring).

    In connection with Registration, all of our families are asked to consider two opportunities to support the Club's fundraising efforts:  (1) an optional, tax-free Donation that can be made along with your Fall Ball dues payment, and (2) Corporate Matching Programs (if this applies to you, please let us know and we'll gladly work with you to maximize that opportunity!).

  • APPROVAL TO PARTICIPATE

    I/We do hereby give approval for my/our daughter/guardianship to participate with the Austin High School Girl’s Lacrosse Varsity and Junior Varsity Team (AHS Girls Lacrosse Team).

    HOLD HARMLESS

    I/We do hereby, for myself, my heirs, executors, administrators, waive, release, absolve, indemnify and agree to hold harmless, any and all adults who coach or assist in coaching, the AHS Girl’s Lacrosse Booster Club and its members, officers, board members, other participants, AHS Girls Lacrosse Team players, Austin High School, the Austin Independent School District, the West Austin Youth Association, AHS Boy’s Lacrosse Club and its members, officers, board members, other participants, and any of the above named parties’ representatives, successors, supervisors, sponsors, and/or organizers, for any claims, actions or injuries in connection with the outing/event(s), practices or games related to the AHS Girls Lacrosse Varsity or Junior Varsity Team.

    TRANSPORTATION

    I/We likewise release from liability any person(s), airline, bus company or other transportation service transporting any student, in a privately owned and/or leased vehicle, to and from any activities connected with the AHS Girls Lacrosse Team.

     

    RESPONSIBILITY FOR PERSONAL ACTIONS

    I/We also agree that I/we am legally responsible for any/all personal actions taken by my/our child/guardianship related to lacrosse practices, games, events, trips and activities, and agree to be financially responsible for any and all damages, legal fees, and other costs incurred as a result of the actions/behavior of my child/guardianship.

    REMOVAL FROM TEAM ACTIVITIES

    Furthermore, I/we agree that if the above named child’s/guardianship’s behavior is inappropriate, unsafe and/or detrimental to the group, I/we will be contacted immediately to secure a means of removing my/our child/guardianship from the practice, game, event, trip or activity premises. I/We understand that any financial cost incurred as a result of my/our child/guardianship being sent home, are my/our responsibility.

    MEDICAL INFORMATION and EMERGENCY CARE

    MEDICAL INFORMATION. I/We understand and agree that, in order to provide a coordinated system of care, the coaching staff may exchange health care information about a player with his or her physician or other healthcare providers. I consent to allow AHSGLAX medical personnel (i.e. trainer) to contact the player's physician directly to share information or request records pertinent to athletic participation. I understand and agree that medical personnel may share the player's health care information with AISD personnel, in accordance with AISD protocol, in order to provide appropriate attention to the player's health care needs. I/We give permission to release and share all necessary health information. I understand that this information will automatically be shared in emergency situations as necessary.

    EMERGENCY CARE. I/We also give permission to seek any emergency care should the child be involved in any accident or be injured in any practices, games, events, trips, and activities. I/We understand that in any such instance, all attempts will be made to contact the parent/guardian. In the event I/we cannot be contacted, I/we herby give permission to the attending physician to hospitalize, secure treatment for, and to order injection, anesthesia and /or surgery for my child as is deemed medically necessary. Unless otherwise and explicitly instructed, it is permissible that my/our daughter/guardianship be given aspirin/pain medication/cold medicine by either medial personnel or other adults responsible during AHS Girls Lacrosse Team events.

  • All students participating in AHSGLAX are required to obtain a new physical dated after April 15, 2021 prior to participating in any practice or activity for the 2021-22 school year.   • INFECTIOUS DISEASE LIABILITY WAIVER

  • AUSTIN HIGH SCHOOL GIRLS LACROSSE INFECTIOUS DISEASE WAIVER OF LIABILITY, RELEASE OF CLAIMS, INDEMNITY AND HOLD HARMLESS AGREEMENT

    In consideration of being allowed to participate in any and all Austin High School Girls Lacrosse (“AHS Lax”) practices, skill sessions, scrimmages, games, tournaments, related events and activities (collectively referred to herein as, “Activities” and individually as an “Activity”), the undersigned acknowledges, understands, and agrees to all of the following agreements and provisions contained in this Infectious Disease Waiver of Liability, Release of Claims, Indemnity and Hold Harmless Agreement (the “Agreement”):

    1) The novel coronavirus (“COVID-19”), has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact and is wide-spread across the United States of America, including the State of Texas.

    2) I understand the hazards of COVID-19 and other infectious diseases such as MRSA and influenza and am familiar with the Centers for Disease Control and Prevention (“CDC”) guidelines regarding COVID-19. I acknowledge and understand that the circumstances regarding COVID-19 are changing from day to day and that, accordingly, the guidelines of the CDC, State of Texas, the University Interscholastic League, Travis County (and surrounding counties), along with the rules and regulations of the owners and leaseholders of the premises where AHS Lax conducts its Activities, including West Austin Youth Association and Austin Independent School District (the “Facility Owners”) are regularly modified and updated, and I accept full responsibility for familiarizing myself and complying with the most recent updates of such rules, regulations and guidelines.

    3) I understand participation in any or all AHS Lax Activities includes possible exposure to and illness from infectious diseases including but not limited to COVID-19, MRSA, and influenza. While AHS Lax has put in place preventative measures to reduce the spread of COVID-19 and other infectious diseases, AHS Lax cannot guarantee that you, your player or your family members will not become infected with COVID-19 or any other infectious disease. Further, attending any AHS Lax Activities could increase your (parent/guardian) risk, your family members’ risk and your player’s risk of contracting COVID-19 or any other infectious disease. While compliance with AHS Lax’s, the CDC’s, the State of Texas’, and Travis County’s rules, regulations and guidelines may reduce the risk of contracting COVID-19 or any other infectious disease, the risk of serious illness and death does exist.

    4) To the best of my knowledge, my player and myself are of good health, are not immune compromised and can fully participate in any AHS Lax Activities as a player and/or spectator. Notwithstanding the risks associated with COVID-19 or any other infectious disease, which I readily acknowledge, I hereby voluntarily choose to participate in any and all AHS Lax Activities as a spectator and/or player and to enter any and all premises used by AHS Lax for its Activities knowing that any of the Activities may be hazardous to me. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, DAMAGE, PERSONAL LOSS, ILLNESS, INJURY OR DEATH FROM COVID-19, OR ANY OTHER INFECTIOUS DISEASE, THAT MAY BE SUSTAINED BY ME, MY FAMILY MEMBERS OR MY PLAYER AS A RESULT OF BEING ENGAGED IN ANY AHS LAX ACTIVITIES, WHETHER CAUSED BY THE NEGLIGENCE OF AHS LAX, ITS MEMBERS, BOARD, OFFICERS, COACHES, CONTRACTORS, VOLUNTEERS, AGENTS, EMPLOYEES, OR BY THE FACILITY OWNERS (COLLECTIVELY REFERRED TO HEREIN AS “RELEASEES” OR INDIVIDUALLY AS “RELEASEE”) OR OTHERWISE.

    5) In consideration for participating in or spectating any AHS Lax Activities and for other valuable consideration which is hereby acknowledged, I hereby RELEASE, WAIVE, HOLD HARMLESS, DISCHARGE AND COVENANT NOT TO SUE RELEASEES from any and all liability, claims, costs, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, illness or injury, including death, that may be sustained by me, my family members or my player FROM COVID-19, OR ANY OTHER INFECTIOUS DISEASES, AS A RESULT OF BEING ENGAGED IN ANY OF THE AHS LAX ACTIVITIES, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES OR OTHERWISE.

    6) I shall indemnify, defend and hold harmless the RELEASEES from and against any and all claims, demands, suits, judgments, losses, costs or expenses of any nature whatsoever including, without limitation, attorneys’ fees, costs and disbursements, whether of in-house or outside counsel, and whether or not an action is brought, on appeal or otherwise, arising from or out of, or relating to, directly or indirectly, an infection of myself, my player or my family members with COVID-19, or any other infectious disease, illness or injury.

    7) It is my expressed intent that this Agreement shall bind the members of my family and spouse (if any), if I am alive, and my heirs, assigns and personal representative, if I am not alive, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above RELEASEES.

    8) I understand that AHS Lax (or any RELEASEE) will not be responsible for any medical costs associated with any illness from COVID-19 or any other infectious disease that I, my player or my family members may sustain as a result of participating in any AHS Lax Activities.

    9) I HEREBY KNOWINGLY AND VOLUNTARILY WAIVE ANY RIGHT TO A JURY TRIAL OF ANY DISPUTE ARISING IN CONNECTION WITH THIS AGREEMENT. I ACKNOWLEDGE THAT THIS WAIVER WAS EXPRESSLY NEGOTIATED AND IS A MATERIAL INDUCEMENT TO THE PERMISSION GRANTED BY AHS LAX AND RELEASEES TO BE ON PREMISES AND TO PARTICIPATE IN THE ACTIVITIES.

    10) I hereby further agree that this Agreement shall be construed in accordance with the laws of the State of Texas.

    11) Electronic, email or facsimile signatures on this Agreement shall be construed for all purposes as original signatures.

    12). If any provision of this Agreement is held to be illegal, invalid, or unenforceable under present or future laws effective during this Agreement, the legality, validity, and enforceability of the remaining provisions of this Agreement shall not be affected thereby, and in lieu of each such illegal, invalid, or unenforceable provision there shall be automatically added, as part of this Agreement, a provision as similar in terms to such illegal, invalid, or unenforceable provision as may be possible and be legal, valid and enforceable.

    13). This Agreement may be executed in any number of counterparts with the same effect as if all parties hereto had signed the same document. All such counterparts shall be construed together and shall constitute one instrument, but in making proof hereof it shall only be necessary to produce one such counterpart.

    BY SIGNING THIS Infectious Disease Waiver of Liability, Release of Claims, Indemnity and Hold Harmless Agreement, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing written agreement, have been made.

    If under age eighteen (18), a parent or guardian must sign indicating their assumption of legal and financial responsibility for my participation in any of the Activities; and I execute this Agreement for full, adequate and complete consideration fully intending to be bound by same.

  • 2021-2022 AHSGLAX PROGRAM PARTICIPATION ACKNOWLEDGEMENT

  • We understand the following:

    A student is not allowed to practice in any AHSGLAX in any AHSGLAX activity whatsoever until the registration process is entirely complete.

    Each family is required to participate in fundraising activities beyond a player's Snap Raise activities.

    All players and parents/guardians are required to use Team App for AHSGLAX news and announcements.

    All players are required to complete baseline concussion testing at the beginning of each year.  Concussion testing will be coordinated through AHSGLAX.

  • AHSGLAX PLAYER EXPECTATIONS AND HER ACCEPTANCE OF THESE

  • In signing below, I agree to abide by all policies and rules established for the AHS Girls Lacrosse Team practices, games, activities, and other events.  Should I not be able to maintain the expectations of the adults and my peers, I understand that there will be consequences for my actions, including removal from the activity and being sent hom at my parent/guardian's expense.  Basic rules and expectations include, but are not limited to, the following:  respect for all coaches, referrees, game officials, adult leaders, peers, opponents, and all property; no illegal drugs, alcohol, smoking/vaping, firearms, explosives, or illegal substances or devices; appropriate attire as determined by the coach or other adult leaders is to be worn at all times.  I further state that I am familiar with the requirements set forth in the AHSGLAX Player Handbook.  Other guidelines may be set forth accordingly by coaches and AHSGLAX Booster Club officers.

  • STUDENT TRANSPORTATION LIABILITY WAIVER & INDEMNIFICATION AGREEMENT

  • LIABILITY WAIVER AND INDEMNIFICATION AGREEMENT

    I am the Parent/Guardian of the below-named student or I am the below-named student, in the event such student is 18 years old (the “Student”), and, in each case, I am fully competent to sign this Agreement. I give permission for Student to drive or be driven to lacrosse-related activities, games and practices (the “Activity” or “Trip”). I acknowledge that the nature of the Activity or Trip may expose Student to hazards or risks that may result in Student’s illness, personal injury or death, and I understand and appreciate the nature of such hazards and risks.

    In consideration of Student being permitted to participate in the Activity or Trip, I hereby accept all risk to Student’s health and of her injury or death that may result from such participation, and I hereby release Austin High Girls’ Lacrosse Team, its governing board, officers, coaches, members, parents and representatives from any and all liability to Student, Student’s personal representatives, estate, heirs, next of kin, and assigns for any and all claims and causes of action for loss of or damage to Student’s property and for any and all illness or injury to Student’s person, including her death, that may result from or occur during Student’s participation in the Activity or Trip, WHETHER CAUSED BY NEGLIGENCE OF A FELLOW AUSTIN HIGH GIRLS’ LACROSSE TEAM MEMBER OR PARENT OR AUSTIN HIGH GIRLS’ LACROSSE TEAM, ITS GOVERNING BOARD, OFFICERS, COACHES OR REPRESENTATIVES, OR OTHERWISE.   I further agree to indemnify and hold harmless Austin High Girls’ Lacrosse Team and its governing board, officers, coaches, members, parents and representatives from liability for the injury or death of any person(s) and damage to property that may result from Student’s negligent or intentional act or omission while participating in the described Activity or Trip.

    I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND IT TO BE A RELEASE OF ALL CLAIMS AND CAUSES OF ACTION FOR STUDENT’S INJURY OR DEATH OR DAMAGE TO STUDENT’S PROPERTY THAT OCCURS WHILE PARTICIPATING IN THE DESCRIBED ACTIVITY OR TRIP, AND IT OBLIGATES ME TO INDEMNIFY THE PARTIES NAMED FOR ANY LIABILITY FOR INJURY OR DEATH OF ANY PERSON AND DAMAGE TO PROPERTY CAUSED BY STUDENT’S NEGLIGENT OR INTENTIONAL ACT OR OMISSION.

  • We, the undersigned, agree to the release of academic records, including grades in progress, to the Head Coach of the Austin High School Girls’ Lacrosse Team, for the purposes of:

    Eligibility to participate in Austin High School Girls’ Lacrosse Team Varsity or Junior Varsity games; and

    Consideration of academic awards that may be offered to lacrosse players. We further understand that if award eligibility is determined, this information will be provided during nominations, in confidence, to award committees for deliberative purposes.
