    • Treasurers receive quarterly rebate memos automatically. If another party (e.g. chapter bookkeeper) should also receive these memos, please indicate so in the space below. 
    • All Chapter reporting forms will be sent to both the Chapter Executive Director/Administrator and Chapter President unless otherwise noted.
    • The phone numbers and email addresses of all board members will be displayed on the Chapter website unless otherwise requested.
    • All officers and directors listed on this report are covered by ACC’s Directors’ and Officers’ Liability Insurance Policy. Please refer to the Chapter President’s Manual for details on insurance for Chapters.
