Release form between you (“Onsite Contractor”) and Legacy Tree Genealogists (“Company”). Whereas Onsite Contractor desires to provide services for Company, Onsite Contractor understands that the scope of relationship with Company is limited to an independent contractor position and agrees to the following: (1) Waiver and Release: I, the Onsite Contractor, understand and acknowledge that this Release discharges Company from any liability or claims that I may have against Company with respect to bodily injury, illness, death, or property damage that may result from the services that I provide to Company. (2) Insurance: Further, I understand that Company does not assume any obligation to provide me with health-related assistance, including but not limited to medical health or disability benefits or insurance of any nature. (3) Medical Treatment: I hereby release and forever discharge the Company from any claim which may arise on account of first-aid treatment or other medical services rendered in connection with any emergency during my work as a contractor for the Company. (4) Other: As an Onsite Contractor, I expressly agree that this Waiver and Release is intended to be as broad and inclusive as permitted by the laws of the State of Utah and that this Release shall be governed by and interpreted in accordance with the laws of the State of Utah. I agree that in the event that any clause or provision of this Waiver and Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected.
Non-Disclosure: Onsite Contractor shall keep Company's Confidential Information, whether or not prepared or developed by Onsite Contractor, in the strictest confidence. Onsite Contractor will not disclose such information, including information about Company’s clients, to anyone outside Company without Company's prior written consent, nor will Onsite Contractor make use of any Confidential Information for Onsite Contractor's own purposes or the benefit of anyone other than Company. Onsite Contractor's obligation to maintain the confidentiality and security of Confidential Information remains even after Onsite Contractor's contract with Company ends and continues for so long as such Confidential Information remains a trade secret.
