By signing this form the customer understands that they are responsible to speak of any concerns with the lash extension specialist. I give permission to my lash extension specialist to perform the lash extension procedure we have discussed, and will hold her harmless and nameless from any liability that may result from this treatment. I have accurately answered the questions below including allergies, prescription drugs, or products that I have ingested or used topically. I understand that my lash extension specialist will take every precaution to minimize or eliminate negative reactions. I will consult with my lash tech if I have questions or concerns regarding the service. I agree that this constitutes full disclosure, and it supersedes any previous verbal or written disclosures. I certify that I have read and fully understand that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks, I do not hold Darcy A. Gonzalez or Aura Lash & Beauty LLC. responsible for any of my conditions that were present but not disclosed at the time of this procedure, which may be affected by the treatment performed today. By signing this form, the customer agrees that he/she accepts all information, suggestions, and recommended activities or treatments provided by the Lash Technician. The customer hereby releases this salon and its employees from any liability, claims, damages, and demands that may result in injuries and loss.

Three-week fills must have 45% of lashes for the three-week price. 

Any fill that IS past three weeks is a full set

Cancellations: Please make any necessary changes at least 24 hours prior to your appointment. If you have a last-minute emergency and need to reschedule PLEASE CONTACT US! Every effort will be made to fill your spot. Rescheduled appointments after the 24-hour notice are subject to pay 100% of the total cost of service. A late cancelation or no-show will be charged the full-service cost.

LATE FEE WILL BE CHARGED IF YOU ARRIVE LATER THAN 15 MINUTES

Pictures: Pictures may be taken before, during, and after your service. As the property of Aura Lash & Beauty LLC and may be used for marketing and other training purposes.  

Policy: If you are dissatisfied with the service provided to you, or feel like you have more fall out than usual you are eligible for a free lash fix within the first 3 days after your service was provided if the lash technician can tell there was a problem on their behalf. I understand lash extensions are a personal service offered, and therefore nonrefundable. If I am unhappy with the service received, I will contact my lash professional immediately for resolution.

Any fill that is past three weeks is a full set
