Consent to Photograph /Videotape / Interview Individuals Please read, date and provide your signature on the next pages Terms & Conditions I hereby agree and give my permission for Fit Girl Club, Conscious Market, Downtown Santa Monica and/or partners to record, film,photograph, audiotape or videotape myself (hereinafter collectively referred to as “Works”) and to display, publish or distribute these Works for the purpose of publishing, posting on the Conscious Market, Downtown Santa Monica and Fit Girl Club websites and posting on social media sites. I hereby waive any right to approve the use of these Works now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Works. I understand that the Works may appear in electronic form on the internet or in other publications outside of the Rise Asset Development’s control. I agree that I will not hold Downtown Santa Monica responsible for any harm that may arise from such unauthorized reproduction.

Fee 

The total fee for the booth space at CONSCIOUS MARKET is non-refundable fee per event. Should the event be cancelled due to weather there will be no refund and vendor will have the opportunity to attend free of charge should the event be rescheduled. All fees due at the signing of this contract.  Should FGC cancel due to unforeseen issues, all monies will be refunded by the Fit Girl Club of said area within 30 days of cancellation.                                                                                                                                   If it is the choice of the vendor to cancel attendance a written notice 60 days prior to the event must be submitted to FGC Event Director. 

Setup

 
Vendor to provide their own table, tent, signage or any other items deemed necessary to exhibit a professional appearance. All said equipment will set up and ready to at least 30 minutes prior to the start / opening of registration and will remain open during the entire event.  The Vendor is responsible for having their own people to man and operate their own booth. There is a no plastic policy and an eco-friendly event. It is the responsibility of the vendor to clear and clean up any trash and remove it according to the venue rules and regulations as set by FGC Event Director and in accordance to any venue or city rules and regulations. If the event is being held in a private venue, all guidelines as set by the venue owner will be adhered to.  It is agreed that by signing this contract both parties consent to follow the guidelines as set by the terms of this contract in accordance with all laws governing the state in which it has been executed. This a legal and binding agreement.
