  • I am 18 years old or older and give consent for photos/videos of services to be taken and posted on all social media platforms associated with So Bare Skin for marketing purposes only.

  • Please note that all services can have certain side effects such as skin lifting, redness, swelling, tenderness, etc. I have read the above information and have given an accurate account of the questions and if I have any concerns, I will address these with my Esthetician. I give permission to my Esthetician to perform the procedure we have discussed and will hold her harmless from any liability that may result from this treatment. I agree to adhere to all safety post care including: no peels, tanning, or wet room services; no swimming/spas/hot tubs for up to 48 hours; and all home skin care protocols as recommended by my service provider. I understand that my Esthetician will take every precaution to minimize or eliminate negative reactions as much as possible. 

    Anyone client under the age of 18 will need the permission of a parent or gradian. 

    Cancelation and late Poilcy 

    By signing below, I understand that there is 10 minute grace period. After that, I will be asked to reschedule. I understand that appointments should be canceled or rescheduled at least 2-24 hours ahead. No-shows will be charged a non-refundable fee of $20.
