-This form does not guarantee adoption or that an animal will be placed in your care. Adoptions are in the sole discretion of Pack Lyfe Rescue Inc.

-Your answers will be carefully considered to see what it best suited for the potentially adopted pet.

-Please give careful consideration in adopting an animal. Animals are a long term commitment up to 15 years in some cases. Please make sure your lifestyle allows the time, patience, love and expenses this animal will need over the years.
- All animals will be fully vetted and spayed/neutered before adoption is finalized.

Rescue pets do need some decompression. What are your goals to help let your pet decompress. It takes time for your new pet to adjust. Are you willing to give them time to learn your new home and become part of your family?

By clicking the submit button, I agree to Pack Lyfe Rescue Inc. adoption process, will undergo a homecheck, and interview at the discretion of Pack Lyfe Rescue Inc. By clicking the submit button, I understand Pack Lyfe Rescue Inc. will check my references including veterinary and personal. By clicking the submit button I understand that when the adoption process is completed you have read and abide by the adoption agreement and information in this adoption application is true and not misleading in anyway. By clicking the submit button, I agree my pet will be spayed or neutered and fully vaccinated before adoption is finalized. By clicking submit I agree to follow any Vaccine and neuter/spay schedule that pack Lyfe rescue sets up with their vet hospital. I also agree to promise to take care of my pet to the best of my ability and if I cannot so will return the pet to Pack Lyfe Rescue. I understand there is an adoption donation associated with adoption of a pet from Pack Lyfe Rescue Inc. and that it is tax deductible according to IRS 501(c)3 guidelines. I understand this donation will ensure the organization is equipped to rescue another homeless pet. By clicking the submit button, I understand there is no "cooling off" period, and that if I no longer want or can no longer care for my adopted pet, I agree to notify Pack Lyfe Rescue Inc. BY EMAIL and provide a 14 day period to allow Pack Lyfe Rescue Inc. to make arrangements for my pet to be taken back into rescue. By clicking the submit button, I agree to indemnify and hold harmless Pack Lyfe Rescue Inc. against any losses, lawsuits, claims, injury, damages incurred by me or to any persons or property by my adopted pet, once adoption has been completed. By clicking the submit button, I understand that Pack Lyfe Rescue Inc. will disclose any of the pet's health or behavior issues known by the above named rescue group before adoption is completed. By clicking the submit button, I understand that if I no longer want my pet, or am no longer able to care for my adopted pet, I will be directed to surrender my pet to Pack Lyfe Rescue Inc. and provide transport to where Pack Lyfe Rescue Inc. deems appropriate.
