• Complete and sign the MassTeach application, including open response questions.
• Obtain a copy of your most recent transcript (unofficial copies will be accepted)
• Get a letter of recommendation from a STEM or education professor from your community college. 
  

• Applications will be accepted on a rolling basis through May 2, 2022. Students will be notified within two weeks of their application being submitted about next steps. The final date to submit all materials is May 2, 2022. Submit all materials electronically to Allison Little (alittle@dhe.mass.edu).

Please type in your answers and submit them along with this application and other supplemental documents. Do NOT print and/or send the responses separately.

I understand that if I am accepted into the NSF Noyce MassTeach program, and I choose to join, then I am committing to the following:

• Participate in MassTeach program events and activities, including STEM Transfer Academy.
• Attend university full time.
• Enroll in university courses that will lead to a STEM degree and secondary education license.
• Pursuant upon acceptance, maintain a GPA in accordance to university program policy.
• Notify the MassTeach program coordinator at my university if I withdraw from school or change personal contact information.
• Upon earning my bachelor’s degree and secondary education license, teach a STEM subject in a local high-needs school district for four years; if I do not, then I will have to repay the money that I have received for being a MassTeach Scholar.

Additionally, I understand that it is my responsibility to apply to a MassTeach-partner state university (Massachusetts College of Liberal Arts, Westfield State, or Worcester State I also understand my acceptance into the MassTeach program is contingent on being accepted by one of the three participating state universities.

The MassTeach program is required to perform an evaluation of its activities. This evaluation is associated with (a) participant academic performance, and (b) participant experiences of the program. All data collected for the MassTeach program will be used only to answer the required NSF evaluation questions and will only be shared with the authorized program evaluator (UMass Donahue Institute).

I consent to participate in the evaluation activities associated with the MassTeach program. This participation includes:

• Allowing the MA Depar tment of Higher Education to use my student data from HEIRS in a dataset for the MassTeach program that will then be shared with the UMass Donahue Institute for analysis.
• Filling out an annual progr am-related survey designed by the UMass Donahue Institute and administered by my institution of higher education, the data for which will be added to a MassTeach dataset that includes HEIRS data.
• Par ticipating in an interview with UMass Donahue Institute staff related to answering the NSF-approved research questions.

You must agree to both sets of conditions below in order to submit a MassTeach Application to the Massachusetts Department of Higher Education using this form:

Confidentiality Statement— Under most circumstances, the text of your application will be considered public records, copies of which are available to any member of the public upon request. However, your name, email addresses, and any other information that identifies you will not be disclosed. Furthermore, no part of your application will be provided in response to a request that asks specifically for an application submitted by you.
