APPLICATION TO CHANGE PROGRAM

The information submitted in this application is valid only for SY 2022-2023.
Steps Application to Change Program
1. Apply to JMC Portal via jmc.edu.ph/jmc-portal then tap on Application on Change of Program
2. Select the corresponding link for the course program
3. Fill out the online form including their reason for shifting.
4. Upload the Academic Evaluation Report from the Student Portal
5. Wait for the confirmation from the former Dean that the Change of program has been approved.
6. Wait for the schedule from the Guidance Office for counseling.
7. Wait for the confirmation from the new Dean for the acceptance in the new program.
8. Wait for confirmation from the Registrar that the transaction has been completed.
9. Proceed to the Enrolment
