This is an application form for people and organisations that would like to make use of the Spa GAA clubhouse facilities, for an activity, event or otherwise. All applications must be reviewed before they’re accepted, so make sure you apply well in advance - we require at least 10 days.

All participants involved in the activity or event must also be members of Spa GAA Club, and thus will be covered by the Club’s public liability policy for officially-sanctioned GAA activities on the club’s premises.

As all participants must be club members, you’ll need to provide their details. Please do so by downloading our spreadsheet, filling it out and attaching it below.

Valid Public Liability Insurance (with a minimum limit of €6.5 million) and Employers’ Liability Insurance (if applicable, with a minimum limit of €13 million) with a specific indemnity noted on the policy to the Trustees, Committee and Agents of Spa GAA Club and Cuman Lúthchleas Gael must be furnished with this form. This application (and, if you’re representing an organisation, copy of insurance documentation) must be submitted to Spa GAA Club at least 10 days in advance, in order to allow sufficient time for approval.

The activity/event must comply with all Club rules, terms and conditions (linked below), and statutory requirements. Failure to read the attached terms and conditions does not exempt you from following them.

In signing this document, I fully understand and acknowledge that:
- In applying for use of the Spa GAA Club’s Clubhouse facilities, I am agreeing to the terms and conditions that are supplied with this application form
- I may not attach material (for example, posters) to the walls, or add any permanent fittings to the premises
- Any and all property that I bring into the premises must be removed at the end of each session
- Payment will be made by cash or direct debit within 14 days of receiving an invoice, which will be issued regularly and in accordance with my schedule
