Hello and thank you for taking the time to share your thoughts and experiences of using IAM!
As a reminder, IAM (or the IAM wheel) is the online assessment you go through with your clinical team to identify support and information needs.
This short survey is for users of IAM and designed to help us (Teenage Cancer Trust) understand your experience of using IAM, what you like about it and what could be improved. We are working with a company called inFocus (www.impactinfocus.com) who are helping us undertake the research project.
The survey has questions on your experience of using IAM and will take around 5 minutes to complete (although feel free to take longer if you need to!). We may publish statistics from the survey, as well as any quotes that you choose to share, but we will never publish anything you share with us that would identify you e.g. your name. Your survey answers will be permanently deleted in January 2022.
We ask for your name, contact phone number and email address to help keep you safe. If you share anything with us that causes us concern about your safety or wellbeing, we will want to get in touch with you and we may need to share concerns with agencies who can offer support. We won’t use them for anything else.
We understand that some people may find it hard or upsetting to think about their cancer so it is important to know that you can stop and exit the survey at any time. If you are under 18 years old you might want to fill this in with a parent or carer.
If any questions in the survey make you feel you need support, please contact a member of your clinical team. You can also contact your GP or call 111 if you feel you need urgent mental health support, and in an emergency always dial 999. Childline (www.childline.org.uk) and the Samaritans (www.samaritans.org) are also available 24 hours a day. Please note responses to the survey will be monitored during office hours only.
