Gym Disclaimer Waiver
I, the gym user, understand and am aware that there are significant risks involved in all aspects of physical training. These risks can result in serious injury or death to myself or to other people around me.
I accept full responsibility for the risks that I am exposing myself to and understand the seriousness of this, and that any form of physical training can result in injury or death, including any physical training or classes at Everest Gym.
As mentioned above, these risks and hazards can have serious implications. I need to consider these risks when training or participating in classes at Everest Gym. I understand that I am voluntarily participating in activities or classes offered by Everest Gym.
I fully understand that Everest Gym and their staff members do not accept any liability, claims, demands, or actions from members training or participating in classes at Everest Gym.
The information you have given on your Physical Activity Readiness Questionnaire is correct.
I understand that the staff members at Everest Gym may not be medically trained. If you should have any concerns with your health and fitness you should seek independent medical advice before engaging in any physical activity on our premises.
To the best of your knowledge and belief, you are in good health and not knowingly incapable of engaging in either active or passive exercise and that such exercise would not be detrimental to your health, safety, comfort, wellbeing or physical condition. Further that you will advise us immediately should your health or vulnerability to injury changes.
If as a member I produce an incorrect Physical Activity Readiness Questionnaire (PARQ), or there are any changes in my health that are not communicated to Everest Gym, this agreement will be invalid.

Health & Safety
Please ensure you are using all gym equipment safely and with proper form. Even if you are experienced at using fitness equipment, it is advisable to be inducted on the equipment within the gym. We are happy to offer you guidance and advice, or we can book you in for an induction, at the cost of a single session, or free to members. More information can be found below.
Induction
We offer inductions at Everest Gym for those who want some beginner guidance on some/all of our fitness equipment. If you require an induction, please click the relevant link upon completion of this form or speak to a staff member at reception. If you choose to refuse an induction, then Everest Gym take no responsibility in improper use and/or any injuries caused by improper use of our equipment.
You can book an induction at the cost of a single session, or free to members.
Everest Gym Rules
Please be respectful of other gym users and staff.
Please be considerate of others around you, especially during the busy periods.
Please use a spotter if needed.
Please wear appropriate gym clothing/shoes – no bare feet or flip flops to be worn in the gym.
Please wipe muddy shoes on the door mats provided as you enter the gym.
Please clean equipment after use.
Don’t leave belongings lying around on the gym floor. Please use a locker to store bags.
Re-rack your weights after use.
Please leave the toilets how you expect to find them.
Smoking, vaping or the consumption of alcohol is NOT permitted in the building.
We are not responsible for any of your personal belongings; this includes your car parked in the gym car park.
Avoid dropping heavy weights from a height.
Use proper gym techniques and know your own body’s limits to help avoid injury. Please ask a member of staff if you require any assistance.
Please be respectful of our gym, the rules, and other gym users and team members.
If you fail to abide by these rules, you may be asked to leave, or be removed from the premises.
