As a Christian Preschool, we teach our students a faith-based curriculum, as well as an academic-based curriculum. Based upon your own personal experiences and relationship with Jesus Christ, do you feel you are adequately trained and equipped to teach a faith-based curriculum?*

I also agree to fill out necessary paper work for background check, including, but not limited to, Criminal Record
Check, Arkansas Child Maltreatment Registry, and possibly have a fingerprint check by the FBI. I also agree to the possibility of random drug testing. I understand that an offer of employment with the preschool is contingent upon the receipt of background information.
I authorize Learn and Play Enrichment Academy to contact my former employers, and I authorize references and former employers to disclose to Learn and Play Enrichment Academy all of my employment records, performance reviews, evaluations, and all other matters related to my work ethic and/or personal character. I waive my right to ever personally view any reference given to Learn and Play Enrichment Academy.
If hired, I agree to take 15 hours of child care classes each year, and obtain my First Aid/CPR Certification, on my own time, at my own cost, without compensation.
