to participate in children and youth ministry activities at and sponsored by Trinity Christian Reformed Church of Grandville, MI, also known as The Refuge. This includes permission for my child/children to go outdoors, or to go offsite with their teachers/leaders with advanced notice to parents. I also give permission for Refuge volunteers/staff to seek medical care for my child/children in case of emergencyand I am also including emergency contact info below.

I also give my child/children as listed above permission to be included in photographs that may be taken and then posted or projected in church or posted on The Refuge website or facebook page.

I also give my child permission to eat any food and drink that is served, with the exception of:
