If you are nominating more than one faculty member, please complete a separate form for each nominee.
Responses to all items are required to submit the form. When you finish the form, please click submit to send the information to the Office of Research. If you do not receive a thank you pop-up, your form was not received. If you missed the pop-up, you should also receive an email, although these sometimes end up in spam filters.
