Behavior Tails, llc will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. However, to the extent that Behavior Tails, llc is insured for any unintentional or negligent errors, omissions, or incorrect assertions, Behavior Tails, llc will be responsible for any such acts or omissions, but only to the extent of such insurance. I have been told by Behavior Tails, llc and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others, and consequently I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify and hold harmless Behavior Tails, llc of any and all claims of injury, expense, costs, or damages caused by my dog. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

I authorize Behavior Tails, llc to take my dog into public spaces during his/her stay for the purpose of training my dog.

I authorize emergency medical care to be provided for my dog(s) by an appropriate veterinarian to be determined by Behavior Tails, llc in the event the my regular veterinarian is not available or that closer care is required. I will reimburse Behavior Tails, llc for any charges related to emergency care, including office visits, procedures, medications, surgeries, etc.

I authorize Behavior Tails, llc to administer or seek 1st aid and resuscitative care for my dog(s) as determined appropriate by Behavior Tails, llc and I agree to indemnify and hold harmless Behavior Tails, llc for all and any results thereof.

Payment Policy:

I agree to pay the full cost of services as stated in the payment information and agreement section of this contract. I understand that I am responsible for the full cost and that there are no refunds given after services are rendered.

Cancellation Policy:

For board and train services, notice of cancellation is required 5 business days in advance to prevent receiving a cancellation fee. The cancellation fee is half of the agreed upon services.

Grooming Policy:

I authorize Behavior Tails, llc to groom or seek professional grooming while my dog is in the care of Behavior Tails, llc if the trainer sees it as necessary. Behavior Tails, llc only offers basic baths with towel drying unless otherwise agreed upon additional services. Additional services including nail trimming must be scheduled ahead of time and have an additional cost.

The person signing this form must be over 18 years of age and the legally responsible party for this animal and by submitting this form they are agreeing that all the information listed is accurate and that you have disclosed any history of aggression or injury that has been caused by the dog.
