K9 Kampus Boarding Agreement & Notice

This form is an online version of the boarding agreement you have completed at drop off in the past. By completing the form ahead of time you will make the drop-off process much quicker. Boarding Drop-off times are: Monday - Friday 8:30am-11am Saturday & Sunday 9:30am-11am (Boarders dropped off after 11am will be charged $5/hr after 11am and $25 after 1pm)*

Boarding Pick-up times are: Monday - Friday 8:30am-3pm Saturday & Sunday 9:30am-3:pm You may have a longer wait time if picking up earlier than 8:30am and later than 3pm because those are our busy daycare drop-off and pick-up times Monday thru Friday. Sunday we don't open our doors until 9am and we close at 4pm. Dropping off or picking up at any other time must be pre-arranged and additional fees will apply*

Important information to know and understand on the K9 Kampus Deposit requirements and Cancellation Policy:

Read Carefully:

Please note that our overnight staff does not have access to the computer/check out system and therefore you must pre-arrange after hour pick-ups for the time you would like to pick-up. We also must have your credit card information on file, and a 50% Non-Refundable boarding deposit must be received when the reservation is made. A 72 hour notice of cancellation or change in schedule must be provided to receive a credit of deposit towards future services at K9 Kampus. A deposit for a reservation cancelled with less than 72 hours notice cannot be applied to any other reservation. A cancellation received with less than 72 hours notice WILL result in a forfeit of deposit for that scheduled stay. Parents are responsible for any destruction of property, equipment and/or injuries to another dog or person caused by their dog.

K9 Kampus Fee Structure & Example below:

K9 Kampus will do whatever we can to accommodate your schedule with the special accommodations and convenience options we offer. Below is a sample description of our fee process.*

Boarding is based on a 24 hours period from the time you enter the facility.
On the day you pick up, if it is later than the time that you dropped off you will be charged a daycare rate according to the time at check out.
After-hour pickups will be processed the following morning. Your receipt will be emailed to you.
After-hours pick-up fees: $20/hour/dog (Please note: Fees are subject to change)

Holiday surcharges apply for all designated holiday schedules. Holiday fees are non-refundable & must be paid when reservations are made.

If you decide to extend your dogs stay at the last minute we cannot guarantee you the same suite and fee. We will do our best to accommodate your original reservation request but you may have to pay a higher rate for the accommodations that are available at that time. A “re-suiting” fee may apply.

It’s not unusual for dogs to be stressed when boarding and although K9 Kampus does everything possible to make your four legged family members’ stay as comfortable and stress-free as possible we prefer to be prepared. We would like your permission to use basic methods to reduce stress and eliminate the need for a vet visit for common issues that we are able to address. By initialing below, you give us permission to use these methods without having to call for prior.

Please initial your approval for all methods below that you approve. Please put NO for any methods you do not wish K9 Kampus to provide. An incomplete form will result in K9 Kampus using our best judgment. The following is for dogs only.

Provide instructions on how best to medicate your dog:

Flagyl- used for settling the stomach from vomiting or diarrhea caused by nerves or stress $5each*

Imodium (plain)- for diarrhea- $1each *

Initial if you give us permission to use Imodium. Enter NO otherwise.

Benadryl- used for anxiety or stress, 1mg per pound of weight- $1each *

Initial if you give us permission to use Benadryl. Enter NO otherwise.

Activia- as a probiotic for upset stomach- $2each*

Initial if you give us your permission to use Activia. Enter NO otherwise.

Every effort will be made to contact you using the information you provided, however, in case of emergency, and you are unreachable we require the following information:

Please note that most veterinarians will not treat your pet if you don't provide written approval, by signing below we can show the veterinarian that you authorize K9 Kampus to seek medical treatment for your pet.

I authorize K9 Kampus & Veterinarian to take whatever measures necessary to treat my dog/animal, not to exceed. A $25.00 vet transportation fee will be added to your stay in the event that K9 Kampus must transport/escort your dog to a veterinarian.*

Enter the maximum dollar amount you authorize us to seek medical attention for your pet.

Veterinarians will NOT provide care for your dog if anesthesia is needed and they can not reach you personally to get permission. By signing below you are stating that you authorize K9 Kampus to approve the use of anesthesia to treat your pet in case of emergency. If you do not sign below the veterinarian has the right to refuse care you are not reachable.

I give K9 Kampus and the chosen Veterinarian PERMISSION to place my dog under anesthesia if necessary to treat my dog without having to speak to me personally if I am unreachable while my dog is in the care of K9 Kampus and I will hold K9 Kampus and the chosen Veterinarian and staff harmless of any liability.

Do NOT sign in the box immediately below this statement if you do not give permission for your dog to be put under anesthesia if we can not get in touch with you.

Client Signature must be present in this box for approval: Only enter your signature in this box IF you authorize K9 Kampus and the chosen vet to perform emergency medical attention on your pet including putting them under anesthesia IF we/they can not reach you personally while out of town. Your signature states you release K9 Kampus and the veterinarian from any liability associated with your pet being under anesthesia.

Adding Peanut Butter, Wet Food, Chicken Broth or other additives to food will incur an additional cost unless provided by parents.*

Someone other than myself will be picking up my dog at the end of this board stay. I understand without written approval & identification from the assigned pick-up person K9 Kampus will NOT release my dog to someone. I give the following person permission to pickup my dog from this board stay.
