Hu'Mend Network is a non-profit organization founded by Nikkia Tisaby, aka "Coach Ky".  Coach Ky is on a mission to empower youth and adults with traumatic mindsets - to overcome barriers in their life by using the tools that she has used as a survivor herself.

​One of Coach Ky's most important missions is to break the negative stigma on mental health, especially in the African American community. 

​As one of the most entertaining and effective speakers/performers, Coach Ky equips those who are going through the struggles of traumatic, negative experiences, the resources to overcome and live a redemptive lifestyle.

 Every opportunity to inspire how a person thinks and feels could be a matter of life or death. She addresses the need to help them feel loved, to know their worth, and to believe that healing is possible. She engages audiences and helps them find the strength and hope in their stories. 

Client's Rights

• The client may ask questions on what to expect during and end result of the coaching session.
• The client may decline to proceed with the coaching session as to the techniques which may be conducted by the coach.
• The client may cease to continue coaching sessions anytime and may return to their professional coaching anytime.
• The coach has the right to dismiss the client from the course of the coach.
• The client has the right to review his or her records from the life coach.

 

Right to confidentiality: Within limits provided for by law, all records and information acquired by the coach shall be kept strictly confidential in accordance with the principles of a coach-client relationship. All information will not be shared or revealed to any person, agency, or organization without the prior written consent of the client. The client can raise any concerns and speak with the coach immediately of any concerns provided that the coach is likewise available to discuss matters with the client.

Acknowledgement I have reviewed this Hu'Mend Network Informed Consent Agreement. I likewise understand my Client's Rights set in this form. I accept this agreement and consent to coaching.
