The size of the item must be 15" x 15" x 15" or smaller. It must fit into the locker on one shelf.
I understand that items left beyond the "End Date" I have chosen above will be thrown away or donated.
Level Up Dog Sports is not responsible for any theft, damage, or missing and lost items. I am leaving my item here at my own risk. Leaving valuable or precious items is not recommended.
We require that you label your item with the recipient's name. Please make sure this is visible and legible to avoid confusion.
I have verified that my Start and End Date are not on a day that Level Up Dog Sports is closed, such as a major holiday or national championship event. My recipient knows where to pick up the item and will come during business hours.
I have verified that the quantity selected represents the number of days I will be using the locker rental. The cost is $2/day, including partial days (ex. a rental of 1 hour will be charged the full day's rate). If Level Up Dog Sports has to unexpectedly close at any time, you will not be charged for additional days, if your recipient cannot pick up the items due to closure.
