Off Leash Permission/Waiver of Liability
I, the undersigned Client, give Thank Dog LLC permission to walk and/or exercise my dog(s) off leash during any part of any walk or playtime/exercise activities undertaken. I understand that any time a dog is taken outside for exercise or transported to an area for exercise there is a risk of injury, death, or loss of that dog. The potential danger to a dog is also increased significantly if the dog is permitted to exercise outside off leash and in an area that is not contained.

I hereby agree to waive all claims for damages against Thank Dog LLC, any employee or agent/independent contractor thereof as the result of all injuries to the pet that occurred while Thank Dog LLC provided the requested services so long as the Thank Dog LLC parties are not negligent in providing those services.

I acknowledge that potential harm could occur to my dog if walking or exercising off leash while in the care of Thank Dog LLC. I agree to release and hold harmless Thank Dog LLC parties from all liability should my dog become lost, injured, or otherwise harmed because of off leash walking and/or playing and exercising. I also release and hold Thank Dog LLC harmless should my dog suffer any injury not due to any negligence on Thank Dog LLC part while walking, playing, or exercising off leash. In addition, I will be responsible for all medical expenses and damage resulting from an injury to Thank Dog LLC, any other person or pet caused by my dog.

I will indemnify, hold harmless and defend Thank Dog LLC in the event of a claim or lawsuit by any person or other pet injured by my dog. This includes expenses and reasonable attorney fees arising from any action brought against the Thank Dog LLC parties related in any way to the action of my dog that occurred while providing requested services.

Thank Dog LLC agrees to exercise all due and reasonable care to prevent injury or death to my dog while off leash. However, in the event of injury or death, except those caused by reckless or negligent acts on the part of Thank Dog LLC the company shall not be liable for such injury or death of my dog because of providing off-leash walking, exercising, or playing. Understanding the risks as stated above, I freely and voluntarily enter into this contract of permission & waiver with Thank Dog LLC.

This contract of permission is a full and complete instrument. No other warranties expressed or implied are valid unless stipulated above. I have read this Off-Leash Walking Permission & Waiver Form and understand its contents. I acknowledge the risk of permitting my dog to walk, exercise and/or play off-leash and indicate my authorization and desire to have my dog exercised off-leash. I hereby request that Thank Dog LLC allow my dog(s) to be off leash when walking, exercising and/or playing from this date forward.
